摘    要:目的:观察不同步行强度、起始和持续时间对2型糖尿病患者餐后血糖与胰岛素水平的影响。方法:12名2型糖尿病患者进行每次间隔1周的1次静息和4次运动实验,测定空腹及标准早餐后各时点的血糖及胰岛素;采用三因素两水平正交设计,混合模型分析。结果:运动可显著降低2型糖尿病患者餐后峰值血糖和血糖曲线下面积(GlucoseAreaUndertheserumconcentrationCurve,GAUC),但对胰岛素曲线下面积(InsulinAreaUndertheserumconcentrationCurve,IAUC)影响不显著(P>0·05),其中运动强度为3·3代谢当量(MetabolicEquivalentsMETs)、持续时间40min、血糖峰值前30min运动的峰值血糖显著低于静息值(P<0·05);糖尿病史>5年组和BMI>26组运动实验峰值血糖和GAUC显著降低(P<0·05)。结论:单次步行可显著降低2型糖尿病患者餐后峰值血糖和GAUC,但对IAUC影响不明显;糖尿病病史长、体形肥胖的患者运动降糖效果更显著。

Abstract:Objective To explore the effects of exercise intensity,duration and postprandial exercise initial time on postprandial glucose and insulin homeostasis in patients with type 2 diabetes mellitus. Methods 12 patients with type 2 diabetes mellitus underwent 4 exercise tests and 1 sedentary test within 5 weeks. Blood samples were drawn at fasting and postprandial 60minutes, 90minutes, 120minutes and 180minutes to detect the contents glucose and insulin. The three factors of exercise at two levels orthogonal design and the linear mixed models analysis were used. Results Both the peak plasma glucose and GAUC(glucose area under the serum concentration curve)decreased significantly (P<0.05) after exercise, while IAUC (insulin area under the serum concentration curve)remained unchanged(P>0.05); and the decreases of peak plasma glucose were significant (P<0.05) in the postprandial 40 minutes exercise group, 30 minutes exercise prior to peak plasma glucose group, and exercise group at 3.3MET. The peak plasma glucose and GAUC were decreased significantly after exercise in subjects of DM>5 years and BMI>26(P<0.05).Conclusions Single bout of exercise can significantly decrease the peak plasma glucose and GAUC but the IAUC in patients with type 2 diabetes,especially in those with longer clinical history and greater body weight.
