摘    要:目的:研究不同指南要求下女性旁观者胸外按压质量与疲劳程度。方法:招募非医学专业女大学生36人,将其随机分入AHA 2005组和AHA 2010组,分别按照2005和2010年心肺复苏指南要求授课及考核,考核合格后进入研究。进入研究的受试者进行8 min单纯胸外按压,在按压前、后记录受试者基本数据,按压过程中记录按压深度、频率及主观疲劳程度评分。结果:两组按压深度均未达到指南推荐要求,且随按压时间出现下降趋势,2010组在第1、2 min与2005组比明显差异(P0.05);心率、收缩压及指尖乳酸在各组中的按压前后均显著增高,组间比较有明显差异(P0.05);主观疲劳评分在第3 min后2010组明显高于2005组(P0.05)。结论:对非医学专业女性受试者,遵循AHA 2010指南要求的按压质量并不显著优于AHA 2005,甚至在长时间按压过程中加剧操作者疲劳。

Abstract:Objective:To investigate the quality of compression and rescuer fatigue in non-medical female bystander under different international guideline for CPR. Method:36 female volunteers were randomized into AHA 2005 and AHA 2010 group. Subjects entered the study after educated the Hand-only CPR course and pass the final exam. All subjects were asked to finished a 8-min hand-only CPR and baseline characteristic was recorded pre- and post-CPR. Depth, frequency and rating of perceived exertion were recorded every minute during CPR. Results: Compression depth in both groups cannot reach recommendation of international guideline, in which display a decline tendency with time. In the first one and two mins, depth in 2010 was significantly more than that in 2005. Heart rate, systolic blood pressure and lactate significantly increased pre- and post-CPR inside and between groups. RPE was higher in 2010 since 3 mins than that in 2005, all P 〈 0.05. Conclusion: For non-medical female bystander, quality of compression in the guideline of AHA 2010 is not better than that in AHA 2005, even the former might increase rescuer fatigue simultaneously.
