血清胆碱酯酶、淀粉酶、血糖及肌酸激酶同工酶水平与急性有机磷农药中毒程度的临床研究

摘    要:目的:探讨急性有机磷农药中毒(AOPP)患者血清胆碱酯酶(ChE)、淀粉酶(AMS)、血糖(BS)及肌酸激酶同工酶(CK-MB)水平与病情严重程度的关系。方法:将AOPP患者250例(中毒组)按照临床中毒程度分为轻度中毒组、中度中毒组和重度中毒组,并选取年龄匹配的健康体检者为对照组,在AOPP患者入院后立即采静脉血3 mL分别检测ChE、AMS、BS及CK-MB。结果:中毒组血清AMS、CK-MB及BS水平均显著高于对照组(P0.05),ChE水平显著低于对照组(P0.05);随着中毒程度的加重,血清AMS、CK-MB及BS水平递升(P0.05),且不同程度中毒组间均有明显差异(P0.05),而ChE下降水平则无显著性差异。结论:检测AOPP患者血清ChE、BS、AMS及CK-MB对判断AOPP严重程度及指导临床治疗具有重要意义。

Abstract:Objective:To study the relationship of severity on patients with acute organophosphorus pesticide poisoning (AOPP) and levels of serum cholinesterase (ChE), amylase (AMS), blood sugar (BS) and creatine kinase isoenzyme (CK-MB). Methods:250 patients with AOPP (poisoning group) were divided into light poisoning (LP), moderate poisoning (MP) and heavy poisoning (HP) groups according to the degrees of poisoning, and healthy subjects of the same age as control group. ChE, AMS, BS and CK-MB were respectively detected after admission 3ml of venous blood. Results: The levels of serum AMS, CK-MB and BS in poisoning group were significantly higher than those in the control group (P 〈 0.05),while the level of ChE in poisoning group was obviously lower than that in the control group(P 〈 0.05). With the aggravation of poisoned, the levels of AMS, CK-MB and BS raised progressively (P 〈 0.05), and there were significantly differences among the three groups(P 〈 0.05),but the decline of ChE had significant difference. Conclusion: There are import significance in judging of the degree of AOPP and guiding clinical treatment for patients with AOPP by determination of ChE, AMS, BS and CK-MB.
