基底节区脑出血后相关神经纤维继发性损害的磁共振研究

摘    要:目的:探讨应用磁共振弥散张量成像(DTI)技术动态观察基底节区脑出血后神经纤维的顺行性、逆行性变性的过程。方法:选取2011年1月-2012年6月收治的单侧基底节区出血病灶患者10例为研究组,分别在发病的第1周、1月、3月进行DTI检测,得到半卵圆中心、放射冠、内囊、大脑脚、脑桥等部位的部分各向异性图(FA)并测量其值,并与正常对照组10例比较。结果:在出血灶同侧近端纤维束行程上和远端锥体束经路上都可出现清晰可见的信号减弱区,从发病的第1周至3月FA值明显降低。结论:基底节区脑出血除了原发部位的损伤,还可以出现其近端、远端纤维的继发性变性改变。

Abstract:Objective: To investigate the time course of anterograde and retrograde degeneration in neural fiber through diffusion tensor imaging (DTI). Methods: 10 patients with a recent unilateral internal basarl ganglia area cerebral hemorrhage as research group were evaluated with DTI at the first week, the first month and the third months respectively from Jan 2011 to Jun 2012. Fractional anisotropy (FA) was measured at centrum semiovale, corona radiate, internal capsule, cerebral peduncle, and pons levels and were compared with normal control group (n=10). Results: Compared with the control group, the FA values of hemorrhage side in the ipsilateral proximal fiber and distal portion of pyramidal were significantly decreased at the first week, the first month and the third months. Conclusion: Subcortical cerebral hemorrhage may result in anterograde degeration and retrograde degeneration in faber tract through the primary lesion.
