摘    要:背景与目的:位于脑十、松果体和丘脑区的颅内病变位置深在,又属于脑重要功能区,手术风险高,全切除难度大,且多为恶性,治疗较为棘手。本文结合我们的临床病例,探讨应用OUR-XGD旋转式伽玛刀治疗手术高风险区颅内病变的疗效。方法:回顾性分析我院近5年来58例经MRI影像证实的手术高风险区颅内病变的伽玛刀治疗临床资料。结果:松果体区5例,局部控制率84.0%,改善率13.5%,无效率2.5%。脑干区39例,局部控制率84.0%。,恶化率6%,死亡率10%。丘脑区14例,局部控制率71.43%,改善率14.28%,无效率7.14%,加重率7.14%。结论:OUR-XGD旋转式伽玛刀是治疗手术高风险区颅内病变又一种有效的方法,该方法安全、微创、可极大地保留脑功能,可达到对恶性病变可控制病灶、延长生存时间、提高生存质量的目的。

Abstract:BACKGROUND & OBJECTIVE: Intrarranial lesions deep-sited or located in functional areas are still challenge to neurosurgeons. This study was to investigate the efficacy of Gamma knife radiotherapy for intracranial lesions deep-sited or located in functional areas. METHODS: Fifty-six patients with intracranial lesions deep -sited or located in functional areas were treated with OUR -XOD rotating gamma knife in the past 5 years in our center were analyzed. RESULTS: For tumors in pineal region, the local control rate and ineffective rate were 66.67% and 33.33%, respectively. For tumors in brain stem, the local control rate, deterioration rate and rate of mortality were 79.48%, 12.8% and 7.69%, respectively. For tumors in thalamus. the local control rate, improvement rate, ineffective rate and deterioration rate were 71.43%, 14.28%, 7.14% and 7.14%, respectively. CONCLUSIONS: OUR-XGU rotating gamma knife is an effective and minimally invasive method to treat intracranial lesions deep-sited or located in functional areas.
