摘    要:目的:研究龋病发展过程中人牙髓内感觉神经纤维降钙素基因相关肽(CGRP)和P物质(SP)的变化特点,以探讨龋病病变程度对牙髓神经纤维的影响。方法:用免疫组织化学染色法标记不同程度的龋病牙髓中CGRP、SP并进行定量研究。结果:4组牙髓中CGRP数量随龋病进展而增多,各组之间比较均有显著差异(P<0.05);牙髓中SP数量中龋时明显增多(P<0.05);而深龋时反而减少。结论:正常牙髓中存在SP、CGRP,其数量与龋坏深度呈正变关系,龋坏早期,神经肽对龋源性刺激做出了反应,随着龋坏的发展,局部免疫应答增强。龋源性刺激可能引起末梢神经纤维释放SP、CGRP。

Abstract:Objective: To study the immunohistochemical changes of sensory neuropeptides positive fibers in human dental pulp of carious teeth. Methods: According to the degree of carious lesion ,34 samples of dental pulp were divided into normal (S0), superficial carious(S1), intermediate caries (S2) and deep caries (S3) groups. The sensory neuropeptides is immunohislochemically examined with calcitonin gene-related peptide(CGRP) and substance P (SP) anti-rabbit monoclonal antibodies. Results:The number of CGRP positive fibers was different between the 4 groups (P<0.05) .The number of SP in S2 group was the largest among the 4 groups (P< 0. 05). Conclusion: There are CGRP and SP positive fibers both in normal and carious dental pulp. In the early period of carious,CGRP and SP have a potential of actively responding to carious-derived stmuli,the intensity of local immune response strongly increases during the carious process. Carious-derived stmuli may induce the release of SP and CGRP from the terminals of pulpal nerve fiber.
