摘    要:目的了解多重耐药菌导致恶性肿瘤患者发生医院感染的现状,为采取有效控制措施,降低和控制多重耐菊菌对恶性肿瘤患者所造成的危害,提供科学的依据。方法采用回顾性调查方法,对我院2002~2006年收治恶性肿瘤患者的医院感染情况进行调查,筛选出多重耐药菌感染者,对结果进行分析。结果发生多重耐药菌感染的恶性肿瘤患者129例,占恶性肿瘤患者医院感染的17.39%,主要以肺癌、食管癌、鼻咽癌和白血病患者为主,感染部位主要是呼吸道.占70、34%;分离出多重耐药株145株,其中C-杆菌99株,G^+球菌46株;排在前5位的细菌依次为大肠埃希菌、铜绿假单胞菌、肺炎克雷伯菌、金黄色葡萄球菌、凝固酶阴性葡萄球菌;对常用抗菌药物有很高耐药性,由多重耐药菌导致的死亡高达45%。结论必须严密监控多重耐药菌引发的恶性肿瘤患者的医院感染病例,切实落实消毒隔离措施,加强对呼吸道的护理,促进抗菌药物的合理应用,以降低和控制多重耐药菌对恶性肿瘤患者的危害。

Abstract:Objective To understand the infectious status of drug- resistant bactgeria in hospital infections among tumor patients. Methods From Jan. 2002 to Dec 2006, 7202 malignant tumor patients with nosocomial infection in the hospital were reviewed. Results The infection rate of multi - drug resistant bacteria in the malignant tumor inpatients main suffering from lung cancer, esophageal cancer, nasopharyngeal cancer and leukemia was 17.39%. The main site of infection was respiratory tract accounted for (70.34%. Among 145 strains of multi- drug resistant bacteria ,99 strains were gram-negative bacteria and 46 strains were gram - positive bacteria. The main species of bacteria were E. coli (24), Pseudomonus aeruginosa (24), Klebsiella pneumoniae (20), Staphylococcus aures (19) and eoagulase negative Staphylococcus epidermidis. All the isolates showed high resistance to antibiotics and the fatality rate was as high as 45%. Conclusion It is essential to monitor the status of hospital infection in inpatients with multi - drug resistant bacteria and strict measures of isolation and sterilization, rantional use of antibiotics must be taken to reduce hospital infection.
