特发性全面癫痫的元记忆状况分析

摘    要:目的 探究特发性全面性癫痫患者的元记忆状况.方法 选取自2008年1月至2016年1月于我院进行治疗的198例特发性全面性癫痫患者作为观察组,另随机选取198名在年龄、性别及教育程度方面与观察组相匹配的健康人作为对照组.于患者发病半年后通过神经心理学背景测试、知晓感判断(FOK)、蒙特利尔认知功能测试(MoCA)、行为记忆检测对受试者的元记忆状况进行分析.结果 观察组与对照组的MoCA结果比较差异均无统计学意义(P>0.05),而在数字广度测试正序DS和逆序DS、词语流畅性测试(VFT)及Stroop效应中两组比较差异均具有统计学意义(P<0.05);在FOK再认成绩、判断准确率、FF、FR(低估)成绩方面两组比较差异均有统计学意义(P<0.05),而在FOK线索回忆成绩和RR、RF(高估)成绩方面两组比较差异均无统计学意义(P>0.05);观察组患者的行为记忆正常者(43.43%)明显低于对照组(77.27%),行为记忆中度障碍者(20.71%)明显高于对照组(7.07%),行为记忆重度障碍者(16.16%)明显高于对照组(0),差异均具有统计学意义(P<0.05),而观察组患者的行为记忆轻度障碍者为19.70%,与对照组的15.66%比较,差异无统计学意义(P>0.05).结论 特发性全面性癫痫患者的执行功能障碍可能是导致其情景记忆监测能力受损的主要机制,同时患者的语义记忆监测相对保留,提示情景记忆监测与语义记忆监测可能依赖不同的神经机制.

Abstract:Objective To explore the metamemory status of patients with idiopathic generalized epilepsy. Methods From Jan. 2008 to Jan. 2016, 198 patients of idiopathic generalized epilepsy treated in our hospital were en-rolled as the observation group, and 198 healthy people with matched age, gender and education level were selected as the control group. The metamemory status of the subjects was analyzed by neuropsychological background test, feeling of knowing (FOK), Montreal Cognitive Assessment (MoCA), and the behavioral memory test. Results The observa-tion group and the control group showed no significant difference in MoCA results (P>0.05), but they had significant dif-ferences in the digital span test positive DS and reverse DS, verbal fluency test (VFT) and Stroop effect (P<0.05). The recognition, accuracy, FF, and FR (underestimated) scores in FOK were significantly different between the two groups (P<0.05), while cued recall, RR, RF (overestimated) scores in FOK showed no statistically significant differences (P>0.05). The observation group showed statistically significant differences with the control group in normal behavioral memory (43.43%vs 77.27%, P<0.05), moderately impaired behavioral memory (20.71%vs 7.07%, P<0.05), severely im-paired behavioral memory (16.16%vs 0, P<0.05), but not in slightly impaired behavioral memory (19.70%vs 15.66%, P>0.05). Conclusion In patients with idiopathic generalized epilepsy, the episodic memory monitoring ability is impaired, mainly due to executive dysfunction, and semantic memory monitoring is relatively retained, suggesting that episodic memory and semantic memory monitoring may rely on different neural mechanisms.
