血液透析患者血清hs-CRP、Hcy与颈动脉粥样硬化的相关性研究

摘    要:目的:探讨维持性血液透析(MHD)患者血清超敏C反应蛋白(hs-CRP)、同型半胱氨酸(Hcy)与颈动脉粥样硬化的关系。方法选择2014年6月至2015年12月间在我院肾脏风湿科治疗的72例MHD患者作为研究对象,检测患者血清hs-CRP、Hcy水平,并用超声测定颈动脉内膜中层厚度(IMT),其中IMT增厚组57例,IMT正常组15例,比较不同颈动脉IMT患者血清hs-CRP、Hcy水平,并分析其相关性。结果 IMT增厚组患者血清hs-CRP、Hcy水平分别为(13.43±2.35) mg/L、(27.15±3.69)μmol/L,均明显高于IMT正常组的(7.81±1.37) mg/L、(17.17±2.24)μmol/L,差异均有统计学意义(P<0.05);相关性分析显示,MHD患者血清hs-CRP、Hcy水平与颈动脉IMT水平均呈正相关性(r=0.424、0.615,P<0.05)。结论血清hs-CRP、Hcy水平可反应维持性血液透析患者颈动脉粥样硬化程度。

Abstract:Objective To investigate the relationship between serum high-sensitive C reactive protein (hs-CRP) and homocysteine (Hcy) and carotid artery atherosclerosis in patients with maintenance hemodialysis (MHD). Methods A total of 72 cases of MHD patients who treated in Department of Renal and Rheumatism in our hospital from June 2014 to December 2015 were selected as research objects. The serum hs-CRP, Hcy levels were detected. Ca-rotid artery intima-media thickness (IMT) were detected by ultrasound, with 57 cases in IMT thickening group and 15 cases in IMT normal group. The levels of serum hs-CRP, Hcy in patients with different carotid artery IMT were com-pared, and the correlation between them were analyzed. Results The level of serum hs-CRP and Hcy in IMT thicken-ing group were (13.43±2.35) mg/L and (27.15±3.69)μmol/L, which were significantly higher than those in IMT normal group, (7.81 ± 1.37) mg/L and (17.17 ± 2.24)μmol/L, and the differences were statistically significant (P<0.05). Correla-tion analysis showed that serum hs-CRP, Hcy levels in MHD patients were positively correlated with carotid artery IMT (r=0.424, 0.615,P<0.05). Conclusion Serum hs-CRP and Hcy levels can reflect the degree of carotid artery atheroscle-rosis in patients with maintenance hemodialysis.
