亚临床甲状腺功能减退症与冠状动脉粥样硬化性心脏病的相关性

摘    要:目的:探讨亚临床甲状腺功能减退症(SH)与冠状动脉粥样硬化性心脏病的关系。方法回顾性分析我院心血管内科2014年1月至2015年6月期间收治的68例冠心病患者的临床资料,按甲状腺激素水平,将合并与未合并SH的患者分别纳入观察组(n=24)与对照组(n=44),比较两组患者的左室舒张末内径(LVEDD)和左室射血分数(LVEF)水平,以及血浆B型钠尿肽(BNP)水平。结果观察组患者超声心动图指标LVEDD为(58.29±5.42) mm,明显高于对照组的(54.25±6.13) mm,LVEF为(50.44±5.23)%,明显低于对照组的(57.12±6.32)%,差异均有统计学意义(P<0.05);观察组患者血浆BNP水平为(194.62±69.42) pg/mL,明显高于对照组的(161.92±52.16) pg/mL,差异有统计学意义(P<0.05)。结论亚临床甲状腺功能减退症不仅与冠心病的发生密切相关,并可损害冠心病患者的心功能。

Abstract:Objective To investigate the relationship between subclinical hypothyroidism (SH) and coronary atherosclerotic heart disease. Methods The clinical data of 68 patients with coronary heart disease, who admitted to De-partment of Cardiology of our hospital from January 2014 to June 2015, were retrospectively analyzed. According to the levels of thyroid hormone, the patients combined with SH and not combined with SH were respectively included into the observation group (n=24) and the control group (n=44). The levels of left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF) and plasma B-type natriuretic peptide (BNP) of the two groups were com-pared. Results The LVEDD in the observation group was (58.29 ± 5.42) mm, which was significantly higher than (54.25±6.13) mm in control group (P<0.05);the LVEF in the observation group was (50.44±5.23)%, which was signifi-cantly lower than (57.12±6.32)%in control group (P<0.05);the plasma BNP level in the observation group was (194.62± 69.42) pg/mL, which was significantly higher than (161.92±52.16) pg/mL in control group (P<0.05). Conclusion Sub-clinical hypothyroidism is closely related to the occurrence of coronary heart disease, and can damage the heart function of patients with coronary heart disease.
