凝血指标和血小板参数联合检测在肝硬化患者中的临床价值

目的探讨凝血指标（PT、APTT、TT）和血小板参数（PLT、MPV）在肝炎后肝硬化与酒精性肝硬化患者中的临床价值。方法检测50例正常人与35例乙肝后肝硬化、21例丙肝后肝硬化、44例酒精性肝硬化患者外周血PT、APTT、TT、PLT、MPV。结果肝硬化组PT、APTT、TT明显高于对照组（P〈0.05）,丙肝后肝硬化患者PLT比酒精性及乙肝后肝硬化患者明显减少（P〈0.01和P〈0.05）。肝炎后肝硬化患者MPV低于正常组（P〈0.05）,而酒精性肝硬化患者MPV高于正常组（P〈0.05）。结论凝血指标和血小板参数可作为衡量肝硬化患者出血倾向和预后判断的重要指标,而肝炎后肝硬化和酒精性肝硬化患者MPV变化的不一致,能反映血小板减少的原因。

Blood coagulation index and platelet parameters in patients with cirrhosis detection of clinical value

Objective To explore clinical value of cogulative function index（ PT, APTT, TT）and plate let parameters （ PLT, MPV） between alcohol and hepatitis queen liver cirrhosis. Methods Check blood PT, APTT,TT, PLT, MPV among 50 normal persons, 35 second and 21 third queen liver cirrhosis,44 alcohol liver cirrhosis. Results Liver cirrhosis group had obviously higher levels of PT, APTT, TT than control group （ P 〈 0. 05 ）, the third queen liver cirrhosis group had lower PLT levels than alcohol and second liver group（ P 〈 0. 01 and P 〈 0. 05 ）. MPV of hepatitis queen liver cirrhosis patients is lower than normal group （P 〈 0. 05 ）while alcohol liver cirrhosis is higher（P 〈 0. 05 ）. Conclusion Cogulative function index and platelet parameters can be considered as important index of bleeding tendency and prognosis judge. Inconformity of MPV between alcohol and hepatitis 1ueen liver cirrhosis can give cause of PLT declining.
