针灸联合血液灌流治疗尿毒症皮肤瘙痒的临床研究

目的 观察针灸联合血液灌流治疗尿毒症皮肤瘙痒的临床疗效.方法 将46例长期进行维持性血液透析治疗的尿毒症皮肤瘙痒患者随机分为治疗组23例、对照组23例.对照组采用常规血液透析治疗.治疗组在血液透析基础上,予针灸联合血液灌流治疗.比较2组瘙痒症状改善疗效、Duo氏瘙痒评分变化.结果 治疗组23例中,完全缓解16例（69.6％）,好转2例（8.7％）,无效5例（21.7％）,总有效率为78.3％;对照组19例中,完全缓解1例（5.3％）,无效18例（94.7％）,总有效率为5.3％.2组比较,经Ridit分析,U=3.67,P=0.007,P＜0.01,说明治疗组疗效优于对照组.治疗组Duo氏瘙痒评分较本组治疗前减少（P＜0.05）.对照组Duo氏瘙痒评分与本组治疗前比较差异无统计学意义（P＞0.05）.治疗组治疗前后Duo氏瘙痒评分差值与对照组比较差异有统计学意义（P＜0.05）,治疗组优于对照组.结论 针灸联合血液灌流治疗尿毒症皮肤瘙痒具有较好的临床疗效.

Treatment of uremic pruritus with acupuncture combined hemodialysis plus hemoperfusion

Objective To observe the curative effect of acupuncture combined hemodialysis （HD） plus hemoperfusion （HP） in the treatment of uremic pruritus. Methods 46 patients suffering maintenance hemdialysis （MHD） were randomly divided into two groups, the control group （n=23）, treated with hemodialysis and the treatment group（n=23）, treated with hemodialysis combined acupuncture. The improvement of pruritus and Duo＇ s scores were recorded. Results Among 23 cases in the treatment group, there were 16（69.6%） with complete relief, 2 （8.7%） with good effect, and 5（21.7%） with poor effect, and the total effective rate was 78.3%. Among 19 cases in the control group, there were 1（5.3%） with complete relief, and 18（94.7%） with poor effect, and the total effective rate was 5.3%. Ridit analysis showed U=3.67, P=0.007, P〈0.01, which indicated the treatment group was superior to the control group. Duo＇s scores showed significant decrease （P〈0.05） in the treatment group, while no significant difference showed in the control group （P〉0.05）. Compared to the control group, Duo＇s scores showed sifnificant difference in the treatment group （P〈0.05）, the treatment group was superior to the control group. Conclusion The acupuncture combined hemodialysis plus hemoperfusion has a curative effect on uremic pruritus.
