羊水过少胎儿脐动脉血流测定与胎盘病理改变

目的 :探讨羊水过少胎儿脐动脉血流测定的临床价值及胎盘病理变化。方法 :应用彩色多普勒超声 ,对 30例羊水过少孕妇 (羊水过少组 )及 30例正常孕妇 (对照组 )进行脐动脉血流S/D值 (收缩期峰值 /舒张期峰值 )测定 ,并对分娩后的胎盘进行病理分析。结果 :羊水过少组脐血流S/D值≥ 3者明显高于对照组 (P <0 0 1) ;胎盘合体滋养细胞结节及绒毛间质纤维化显著增加 ,血管合体膜显著减少 (P <0 0 1) ,不良围产儿结局发生率增高 (P <0 0 5 )。结论 :羊水过少时 ,胎盘发生明显病理变化 ,脐动脉血流S/D值测定 ,可作为了解羊水过少胎儿胎盘功能和预测胎儿宫内缺氧的指标 ,具有较高的临床价值。

Measurement of Umbilical Artery Blood Flow of Oligohydramnios Fetal and Changes in Placental Pathology

Objective:To investigate the clinical value of umbilical artery blood flow measurement in oligohydramnios fetal and changes in placental pathology. Methods: The S/D ratio of umbilical artery was examined in 30 cases with oligohydraminos pregnancy (oligohydramnios group) and 30 cases of normal pregnancy(control group) diagnosed by colour ultrasound, and the pathological analysis of placenta after delivery was also performed. Results: The S/D ratio of umbilical artery blood flow in oligohydramnios pregnancy was high and it was significantly higher in than those of normal pregnancy (P<0 01); The increased syncytial knot of placental syncytio trophoblast and villus interstitial fibric of oligohydramnios pregnancy, the decreased vasulo-syncytial membrance of oligohydramnios pregnancy (P<0 01), the increased outcome of undeveloped perinatal infants of oligohydramnios pregnancy (P<0 05) were clcarly found. Conclusion: In oligohydramnios, significant pathological changes were found in placenta. Umbilical artery blood flow measurement has the higher clinical value to detect fetal-placental function and to foretell indexes of fetal distress in oligohydramnios.
