非创伤性股骨头坏死与激素、酒精的相关性研究

目的探讨非创伤性股骨头坏死的病因构成及特点,为临床上股骨头坏死的防治提供理论依据。方法对非创伤性股骨头坏死143例(235髋),按病因分组,并对坏死程度、性别比例、激素使用情况以及激素性股骨头坏死的原发病进行分析。结果酒精性48例占33.6%,激素性71例占49.7%,酒精 激素性5例占3.5%,病因不明19例占13.3%。就诊病人以中期较多,女性患者没有因酒精引起的股骨头坏死,而因激素引起的却多于男性。激素性股骨头坏死的原发病涉及多个系统的疾病。结论提高激素可能导致股骨头坏死的意识,定期对这一高危人群进行排查,是早期诊断股骨头坏死、早期治疗的关键;酒精性股骨头坏死的预防要做到少量、间断饮酒,尽量不饮酒。

A study of the correlation between non-traumatic osteonecrosis of the femoral head and steroid or alcoholism

Objective To explore the compositions and character of the etiology of non-traumatic osteonecrosis of the femoral head (ONFH) in order to provide theoretical evidence for the clinical prevention and treatment of ONFH. Methods To group 143 cases(235 hip) with non-traumatic ONFH according to etiology and analyze extent of osteonecrosis?sexual proportion?the application of steroid and the primary disease of steroid-induced ONFH. Results The proportion of alcoholism (48 cases), steroid (71 cases), alcoholism and steroid (5 cases) and etiology unknown (19 cases) is respectively 33.6%, 49.7%, 3.5%, 13.3%. Most of the patients are in the intermediate stage. None of the female patients was induced by alcoholism, but there were more steroid-induced cases found in female patients than that were in male patients. Diseases of multisystem were involved in the steroid-induced ONFH. Conclusions The key points of early diagnosis and treatment of ONFH include enhancing the consciousness that steroid may lead to ONFH and regular examinations of the high-risk group. The prevention of the alcoholism-induced ONFH should be as follow: intermittence or small amounts of alcohol drinking or drinking without any alcohol as far as possible.
