血清葡萄糖-6-磷酸异构酶测定在类风湿性关节中的变化及临床价值

目的探讨葡萄糖-6-磷酸异构酶（G6PI）在类风湿关节炎（RA）诊断中的价值。方法用酶联免疫吸附试验（ELISA）双抗体夹心法检测82例RA患者、68例非RA患者、60例体检健康者血清G6PI、抗环瓜氨酸肽抗体（CCP抗体）、类风湿因子（RF）浓度,。用四格表分析评价G6PI、CCP抗体对RA的诊断效能。结果 G6PI浓度：RA患者为（3.49±2.11mg/L）,非RA患者为（0.136±0.067mg/L）,健康对照组为（0.113±0.043mg/L）。RA患者血清中G6PI浓度显著高于非RA组和健康对照组（P〈0.01）,在RA患者中G6PI的敏感性高于抗CCP抗体和RF（P〈0.05）,而特异性低于抗CCP抗体高于RF（P〈0.05）。相关分析显示RA患者G6PI浓度与类风湿因子（RF）浓度呈显著正相关（r=0.622,P〈0.01）,与CCP抗体无相关性（r=0.203 P〉0.05）。结论 G6PI抗原是独立于抗CCP抗体之外的又一RA早期诊断及判断预后较理想的指标。

The changes and clinical value of glucose-6 -phosphate isomerase in patients with rheumatoid arthritis

Objective To discuss the value of glucose-6 phosphate isomerase （G6PI） in the diagnosis of rheumatoid arthritis（RA）. Methods The serum concentrates of G6PI,anti CCP,RF in 82 patients with RA,68 patients with other rheumatic diseases （non-RA）,60 healthy confrols were determined by ELISA double antibody sandwich method. The diagnostic effects of G6PI and anti-CCP for RA were analyzed by four-column table. Results The serum concentrates of G6PI in patients with RA, non-RA and healthy control group were desperately 3.49±2. 11 mg/L, 0. 136 ±0. 067 mg/L,0. 113±0. 043 mg/L. The serum concentrate of G6PI in patients with RA was obviously higher than that of non- RA group and healthy control group （P〈0.01）. The sensitivity of G6PI in patients with RA was higher than anti-CCP or RF（P〈0.05）. However the specificity of G6PI was lower than anti-CCP,higher than RF（P〈0.05）. The relative a- nalysis displayed that in patients with RA the serum concentrate of G6PI have significantly positive correlation with RF （r=0. 622,P〈0.01） ,whereas has no correlation with anti-CCP（r=0. 203 P〉0.05） . Conclusion G6PI is one of ideal indexes for early diagnosis and prognosis for RA separated from anti-CCP.
