利多卡因治疗重度颅脑损伤后继发性脑损伤的临床研究

摘    要:目的探讨早期静脉注射利多卡因对重度颅脑损伤患者的治疗作用。方法重度颅脑损伤患者60例(格拉斯哥昏迷评分GCS≤8分),随机分为早期静脉注射利多卡因组(观察组)和对照组。在治疗前后对患者均进行GCS评分、颅内压(ICP)测定、头部CT、ECT、TCD检查。结果利多卡因治疗3~5d后即出现颅内压降低、挫伤脑组织血流供应改善、脑水肿减轻,GCS评分在治疗7d、14d后较对照组明显增加(P<0.05)。结论早期静脉注射利多卡因能明显减轻重度颅脑损伤后继发性脑组织损伤的程度,利于神经功能的早期恢复。

Abstract:Objective To investigate the effect of early treatment with lidocaine by intravenous injection on severe craniocerebral injury .Methods Sixty patients with severe craniocerebral injury (GCS≤8) were randomly divided into the control group (treated with routine therapy) and study group(treated with lidocaine). Lidocaine was intravenously administrated continuously for 14 days within post-injury 3 h . All patients were given with Glasgow Coma Scale (GCS),head CT and ECT and TCD examination and measuring brain pressure by spinal puncture.Results After 3~5 days treatment with lidocaine,intracranial pressure(ICP) was significantly decreased,supply of cerebral contusion blood and brain edema was significantly improved,and GCS was significantly increased after 7 and 14 days treatment,compared with control group(P<0.05).Conclusion Early treatment with lidocaine by intravenous injection can alleviate the second brain damage,which is helpful to recover brain function.
