经皮穿刺肺空洞灌注给药治疗空洞型初治菌阳肺结核(附166例报告)

摘    要:目的评价经皮穿刺肺空洞灌注给药治疗空洞型初治菌阳肺结核的临床效果。方法选择我院空洞型初治菌阳肺结核166例,随机分为观察组(72例)和对照组(94例)。2组全身抗结核治疗方案均为3HRZE(S)/6HR,观察组另给予经皮穿刺肺空洞内给药。于第一月末、第二月末和疗程末观察2组痰菌转阴、肺内病灶吸收、空洞变化情况及不良反应。结果观察组痰菌阴转率,肺内病灶吸收好转率及空洞闭合好转率均优于对照组(<0.05);2组均无严重不良反应发生。结论经皮穿刺肺空洞灌注给药是治疗空洞型初治菌阳肺结核的有效和安全的治疗方案,对控制传染源和促进空洞闭合有很好的作用,值得临床推广。

Abstract:Objective To study and evaluate the clinical effects of cavitary pulmonary tuberculosis of initial treatment treated by percutaneous pulmonary paracentesis and perfusing chemotherapy. Methods 166 patients with cavitary pulmonary tuberculosis of initial treatment were randomly devided into two groups. 72 cases,as therapy gro-up,were treated by percutaneous pulmonary paracentesis,whileother 41 cases,as controlgroup,were not.They were all treated with chemotheraphy,3HRZE(S)/ 6HR. Results The sputum negative rate,the lesion absorption rate and the cavity closing rate in the therapy group were allhigher than in the controlgroup ( <0.05). There was a significant difference after the first and second month respectively ( <0.01). No obvious severe adverse effects were found with percutaneous pulmonary paracentesis. Conclusion Percutaneous pulmonary paracentesis and perfusing chemother-apy is an effective and safe treatment approach for patients with cavitary pulmonary tuberculosis of initial treatment,therefore,it could be approached extensively in clinical practice.
