摘    要:目的采用C反应蛋白(CRP)联合降钙素原(PCT)针对早期感染性疾病进行预测,为临床提供依据。方法选择我院2011年12月-2012年5月就诊的120例疑似感染者作为观察对象,按照感染类别分为细菌感染组、非细菌感染组、对照组;感染组分为全身感染组与局部感染组;采用ELISA免疫法和比浊法测定所有患者的PCT及CRP水平。结果将PCT阈值设定为0.5ng/ml预测感染的敏感性为70.5%,特异性83.2%,阳性预测值83.2%,阴性预测值29.4%;CRP阈值设定为5mg/L时,敏感性为76.5%,特异性66.9%,阳性预测值83.3%,阴性预测值38.5%;全身感染组PCT水平显著高于局部感染组(P〈0.05);细菌感染组PCT水平显著高于非细菌感染组(P〈0.05),各组间CRP水平无显著差异性(P〉0.05),但均明显高于对照组。结论 CRP和PCT均可以用于感染性疾病的诊断,它们的水平可能同感染的发生存在一定相关性,在感染性疾病的早期预测中将两者联合检测结果更为可靠,具有较高的临床运用价值。

The application of CRP joint calcitonin original detection in the early forecasting infectious disease
Abstract:Objective The c-reactive protein(CRP) joint calcitonin original(PCT) for predicting the early infectious diseases,and provide a basis for clinical.Methods Choose from December 2011 to May 2012 120 cases of suspected infections as observation object,according to the infection category divided into bacterial infection group,the bacterial infection group,control group;Infection group is divided into systemic infection group and local infection group;Using ELISA immune method and turbidimetric method determination of all patients PCT and CRP level.Results The PCT threshold set to 0.5 ng/ml prediction infection had sensitivity of 70.5%,specificity of 83.2%,positive predictive value of 83.2%,and negative predictive value,29.4%;CRP threshold set to 5 mg/l,a sensitivity of 76.5%,specificity of 66.9%,positive predictive value of 83.3%,and negative predictive value,38.5%;Systemic infection group of PCT level significantly higher than local infection group(P 0.05);Bacterial infection group is significantly higher than PCT level not bacterial infection group(P 0.05),each group CRP level no significant difference(P 0.05).Conclusion CRP and PCT all can be used in the diagnosis of infectious diseases,their level with possible infection has some correlations in infectious diseases,early prediction will both joint detection result is more reliable,and has high clinical application value.
