针刺外关穴对小脑响应机制的18F-FDG PET脑功能成像研究

摘    要:目的通过针刺健康志愿者右侧外关穴,采用PET脑功能成像技术观察小脑皮质葡萄糖代谢变化,研究针刺外关穴对小脑皮质的影响,探讨针刺穴位小脑的响应机制。方法将18例健康志愿者随机分为右侧外关穴针刺组、右侧外关穴假针刺组和空白对照组,每组6例,全部受试者接受PET脑功能成像检查,所得图像采用SPM8软件包进行处理和分析。结果以空白对照组作为对照,右侧外关穴假针刺组小脑未见激活区域,而右侧外关穴针刺组激活的小脑脑区为左侧小脑扁桃体、左侧小脑后叶、左侧小脑前叶及右侧小脑后叶山坡。以右侧外关穴假针刺组作为对照,右侧外关穴针刺组激活的小脑脑区为左侧小脑前叶山顶及右侧小脑后叶下半月小叶。结论针刺外关穴激活了左侧小脑前叶及双侧小脑后叶,为解释针刺穴位产生治疗作用的机制提供依据。

Abstract:Objective To investigate the effect on the cerebellar cortex of acupuncture at right point waiguan(SJ5) in healthy volunteers and explore the mechanism of cerebellar response to acupuncture by using PET brain imaging to observe the change in glucose metabolism in the cerebellar cortex. Methods Eighteen healthy volunteers were randomly allocated to right point waiguan(SJS) acupuncture, right point waiguan(SJ5) sham acupuncture and blank control groups, 6 persons each. All the subjects receive a PET brain imaging examination. The images were processed and analyzed using Software Project Management Board (SPMB). Results In comparison with the blank control group, no cerebellar regions were activated in the right point waiguan(SJ5) sham acupuncture group, and left cerebellar tonsil, posterior lobe and anterior lobe and right cerebellum posterior lobe declive were activated in the right point waiguan(SJ5) acupuncture group. In comparison with the right point waiguan(SJ5) sham acupuncture group, left cerebellum anterior lobe culmen and right cerebellum posterior lobe-inferior semi-lunar lobule were activated in the right point waiguan(SJ5) acupuncture group. Conclusion Acupuncture at point waiguan(SJ5) activates left cerebellar anterior lobe and bilateral cerebellar posterior lobes, which provide a basis for explaining the mechanism of therapeutic action of acupuncture.
