影像学检查在原发性甲状旁腺功能亢进症术前定位诊断中的作用

摘    要:目的 比较超声、CT、MRI 99mTe-MIBI(甲氧基异丁基异腈)在原发性甲状旁腺功能亢进症(PHPT)术前定位诊断中的价值.方法 回顾性分析同济医院24例手术治疗的PHPT患者的临床资料.结果 超声,CT、99mTc-MIBI双时相显像、MRI诊断符合率各为91%(20/22)、73%(11/15)、94%(15/16)、100%(4/4).结论 超声,CT、99mTc-MIBI双时相显像、MRI在原发性甲状旁腺功能亢进症术前定位诊断中具有重要的价值.超声和99mTc-MIBI核素显像应作为首选的术前定位检查方法,若条件允许可以行其它影像学检查.

Abstract:Objective To evaluate the diagnostic value of US,CT,MRI and 99mTc-MIBI in primary hyperparathyroidism(PHPT).Methods Data of 24 PHPT patients with diagnosis confirmed by postoperative pathology were retrospectively reviewed.Results The preoperative diagnosis in 20 out of 22 cases(91%) undergoing preoperative US of the neck with positive result was verified by operative findings.For CT,MRI and 99mTc-MIBI,the positive rate was 73%,100% and 94% respectively.Conclusion US,CT,99mTc-MIBI and MRI play an important role in the preoperative loci localization in patients with PHPT.US and 99mTc-MIBI should be considered as the first choice for preoperative localization of tumors in patients with PHPT.US in combination with 99mTc-MIBI is more sensitive and accurate for the localization of parathyroid gland.
