高敏C反应蛋白及红细胞分布宽度预测老年原发性高血压患者左室肥厚的价值

摘    要:目的 探讨高敏C反应蛋白(hs-CRP)及红细胞分布宽度(RDW)预测老年原发性高血压患者左室肥厚(LVH)的价值.方法 选取2013年7月至2015年3月在我院心内科住院及门诊收治的老年原发性高血压患者158例为研究对象,所有患者均行心脏超声检查,依据左室质量指数(LVMI)将患者分为左室肥厚组(LVH组,n=72例)和非左室肥厚组(非LVH组,n=86例);检测并比较两组患者的血清hs-CRP、RDW以及相关实验室检查指标.利用二分类非条件Logistic回归分析老年原发性高血压患者左室肥厚的相关因素.结果 LVH组患者血清hs-CRP (6.14±2.20) mg/L]高于非LVH组(3.05±1.13) mg/L](t=11.362,P<0.05),LVH组RDW(14.24±0.89)%]高于非LVH组(12.78 ± 0.73)%] (t=11.019,P<0.05).二分类非条件Logistic回归分析显示,血清hs-CRP (OR=2.024,P<0.05)、RDW (OR=1.243,P<0.05)是老年高血压患者发生左室肥厚的独立危险因素(P<0.05);ROC曲线分析显示,血清hs-CRP、RDW预测老年高血压患者发生左室肥厚的曲线下面积(ROCAUC)分别为0.882和0.823;灵敏度分别为90.1%和87.9%;特异度分别为79.6%和78.1%.结论 老年高血压左室肥厚患者血清hs-CRP、RDW显著升高,且与左室肥厚的发生具有正相关性,临床检测两者指标对于评价高血压患者左室肥厚的发生具有一定的临床意义.

Clinical predictive value of high-sensitivity C-reactive protein and red blood cell distribution width in left ventricular hypertrophy in elderly patients with essential hypertension
Abstract:Objective To explore the clinical predictive value of high-sensitivity C-reactive protein (hs-CRP) and red blood cell distribution width (RDW) in left ventricular hypertrophy (LVH) in elderly patients with essential hy-pertension. Methods A total of 158 patients with essential hypertension in Department of Cardiology in our hospital from July 2013 to March 2015 were selected as the research subjects. All the patients received cardiac ultrasound, and were divided into left ventricular hypertrophy group (LVH group, n=72) and non-left ventricular hypertrophy group (non-LVH group, n=86) according to left ventricular mass index (LVMI). Serum hs-CRP, RDW and related laboratory examination indexes were detected and compared between the two groups of patients. Binary classification uncondi-tioned logistic regression analysis was used to analyze the related factors of elderly patients with essential hypertension left ventricular hypertrophy. Results Serum hs-CRP of LVH group (6.14 ± 2.20) mg/L] was significantly higher than that of non-LVH group (3.05 ± 1.13) mg/L], t=11.362, P<0.05. RDW of LVH group (14.24 ± 0.89)%] was significantly higher than that of non-LVH group (12.78±0.73)%], t=11.019, P<0.05. Logistic regression analysis showed that the se-rum hs-CRP (OR=2.024, P<0.05), RDW (OR=1.243, P<0.05) were the independent risk factors for left ventricular hyper-trophy (P<0.05). ROC curve analysis showed that the area under the curve (ROCAUC) of serum hs-CRP, RDW predic-tion of left ventricular hypertrophy were 0.882 and 0.823. The sensitivity were 90.1% and 87.9%, and the specificity were 79.6%and 78.1%, respectively. Conclusion Serum hs-CRP, RDW in hypertensive elderly patients with left ven-tricular hypertrophy are increased significantly, and they were positively correlated with the incidence of left ventricular hypertrophy. Clinical detection of the two indicators has a certain clinical significance in the assessment of left ventricu-lar hypertrophy in hypertension patients.
