软肝化纤汤治疗对乙型肝炎肝硬化代偿期患者肝功能的影响

目的探究乙型肝炎肝硬化(HBC)代偿期患者采用软肝化纤汤联合抗病毒治疗对患者肝脏硬度、肝纤维化以及肝功能的影响。方法选取河南省中医院肝胆脾胃科2018年6月-2019年10月收治的100例HBC代偿期患者为研究对象,依据随机数字分组原则按照1∶1比例将患者分为对照组(n=50)和联合组(n=50),对照组采用阿德福韦酯抗病毒治疗,联合组采用阿德福韦酯抗病毒联合软肝化纤汤治疗。比较两组患者治疗前后中医症状积分、肝纤维化指标、肝功能指标及影像学指标,比较两组治疗效果和不良反应。结果两组患者治疗后中医症状评分、肝纤维化、肝功能指标较治疗前改善(P<0.05),联合组患者中医症状评分、肝纤维化指标及肝功能指标水平低于对照组(P<0.05);联合组患者治疗后脾脏厚度、脾静脉内径、门静脉内径、肝脏斜径、肝脏硬度等影像学指标水平低于治疗前和治疗后对照组(P<0.05);联合组患者治疗总有效率高于对照组(96.0%vs 80.0%;P<0.05);对照组和联合组不良反应发生率比较,无统计学差异。结论HBC代偿期患者采用软肝化纤汤联合抗病毒治疗可以有效降低患者肝脏硬度,减轻肝纤维化程度,改善肝功能,治疗效果显著。

Influence of Ruangan Huaxian Decoction on liver function of patients with compensated hepatitis B cirrhosis

OBJECTIVE To explore the influence of Ruangan Huaxian Decoction combined with antiviral treatment on liver stiffness,liver fibrosis and liver function of the patients with compensated hepatitis B cirrhosis(HBC).METHODS A total of 100 patients with compensated HBC who were treated in department of liver,gallbladder,spleen and stomach of Henan University of Traditional Chinese Medicine from Jun 2018 and Oct 2019 were recruited as the study subjects and randomly divided into the control group with 50 cases and the combination group with 50 cases in a 1∶1 ratio.The control group was given adefovir dipivoxil antiviral therapy,while the combination group was given aefovir dipivoxil antiviral therapy combined with Ruangan Huaxian Decoction.The traditional Chinese medicine symptom score,liver fibrosis indexes,liver function indexes and imaging indexes were compared between the two groups of patients before and after the treatment,and the curative effects and adverse reactions were compared between the two groups.RESULTS The traditional Chinese medicine symptom score,liver fibrosis indexes and liver function indexes of the two groups were significantly improved after the treatment(P<0.05).The traditional Chinese medicine symptom score and levels of liver fibrosis indexes and liver function indexes of the combination group were significantly lower than those of the control group(P<0.05).The levels of imaging indexes such as thickness of spleen,diameter of splenic vein,diameter of portal vein,liver oblique diameter and liver stiffness of the combination group significantly were lower than those of the control group before and after the treatment(P<0.05).The total effective rate of treatment of the combination group was 96.0%,significantly higher than 80.0%of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the control group and the combination group.CONCLUSION Ruangan Huaxian Decoction combined with antiviral therapy can effectively reduce the liver stiffness,alleviate the liver fibrosis and improve the liver function,and it can achieve remarkable curative effect.
