肾实质切开取石在肾皮质变薄肾结石中的应用

摘    要:目的探讨肾实质切开取石在肾皮质变薄的肾结石中的疗效。方法对28例肾皮质变薄的肾结石患者采用开放性肾实质切开取石,分析手术时间、取石时间、结石清除率、术中出血情况以及术后并发症等。结果采用肾实质切开取石治疗肾皮质变薄的肾结石28例,平均手术时间95 min(含麻醉时间),平均取石时间21 min,结石取尽率为89.3%,平均术中出血量为110 mL,无大出血病例,术后发热16例,给予有效抗生素处理,除1例术后6 d好转外,余均2~3 d后体温恢复正常。术后所有患者均无肾功能损伤,无下肢深静脉血栓形成等并发症。结论肾实质切开取石对于肾皮质变薄的肾结石的治疗是一种安全有效的方法。

Abstract:Objective To explore the effect of cutting renal parenchyma to remove calculus in the treatment of renal calculi with thinner renal cortex.Methods A retrospective study of 28 cases cutting renal parenchyma to remove calculus in the treatment of renal calculi with thinner renal cortex was done,to analyse the time of operation and removing calculus,the bleeding during the operation,the rate of stone clearance,and the complications,etc..Results The mean time of operation was 95 min(including anesthesia time).The mean time of stone removal was 21min.The rate of clearing stone was 89.3%.The mean blood loss was 110 mL.There was no hemorrhea after the operation.16 patients got fever after operation.Their body temperatures returned to normal within 2-3 days after effective antibodies were used except one case in 6 days after operation.Conclusion Cutting renal parenchyma to remove calculus is a safe and effective method to treat renal calculi with thinner renal cortex.
