2型糖尿病患者血管内皮功能紊乱和尿微量白蛋白的关系

摘    要:目的探讨2型糖尿病患者血管内皮功能与尿微量白蛋白的关系。方法收集128例2型糖尿病患者的一般临床资料,并对患者的生化指标和尿白蛋白排泄率(UAER)进行检测,采用血流介导的肱动脉扩张(FMD)法评估其血管内皮功能。结果 2型糖尿病患者的FMD与UAER(r=-0.315,P=0.003)、糖化血红蛋白(r=-0.284,P=0.014)、体质量指数(BMI)(r=-0.210,P=0.040)呈负相关关系,多元回归分析表明UAER和HbA1c是内皮功能的独立影响因素(调整的r2=0.280,P<0.01)。结论尿微量白蛋白是2型糖尿病患者血管内皮功能的独立预测因素。

Abstract:Objective To discuss the correlation between vascular endothelial function and microalbuminuria in patients with type 2 diabetes.Methods 128 patients with type 2 diabetes were enrolled in the cross-sectional study.The clinical data,conducted laboratory tests,calculated urinary albumin excretion rate(UAER)of these patients were collected.Meanwhile,the endothelial function were evaluated by brachial artery flow-mediated dilation(FMD).Results FMD was associated with microalbuminuria(r=-0.315,P=0.003),HbA1c(r=-0.284,P=0.014)and body mass index(r=-0.210,P=0.040).Multiple regression analysis showed that microalbuminuria and HbA1c were the independent determinants of FMD(adjusted r2=0.280,P<0.01).Conclusion Microalbuminuria is the independent predictor of FMD in patients with type 2 diabetes.
