老年胆道良性疾病的外科治疗

目的：探讨老年人胆道良性疾病的手术治疗方法及疗效。方法：回顾性分析了我院自1998年1月-2008年1月收治102例65岁以上老年人胆道良性疾病的手术治疗情况。结果：本组患者行腹腔镜胆囊切除术72例,胆囊切除术11例,胆囊造瘘4例,胆囊切除加胆总管探查＂T＂型管引流术12例,胆总管空肠Roux-Y吻合术3例。术后并发症8例（腹腔镜胆囊切除组无一例发生）,其中切口感染3例,切口裂开1例,肺部感染2例,应激性溃疡出血1例,胆瘘1例。结论：高龄不是胆道手术的禁忌症,选择合适的病例、恰当的手术时机及手术方式,做好围手术期处理是提高手术成功率、降低并发症和病死率的关键。

Surgical Treatment of Elderly Benign Biliarytract Disease

Objective：To investigate efficacy of surgical treatment of elderly benign biliarytract disease. Methods：A retrospective analy-sis 102 cases of hospital since 1998-2008 in January. Results：Laparoscopic cholecystectomy in patients with 72 cases,11 cases of cholecystectomy,Choledochostomg 4 cases of fistula,cholecystectomy plus common bile duct ＂T＂ -tube drainage in 12 cases,common bile duct jejunum Roux-Y anastomosis in 3 patients. Complications in 8 cases （ laparoscopic cholecystectomyGroup and no case） ,3 cases of wound infection,wound dehiscence in 1 case,2 cases of pulmonary infection,stress ulcer bleeding in 1 case,Conclusion：The elderly are not biliary surgery selection of suitable cases,the appropriate timing of surgery and surgical approach,perioperative management is doing to improve the success rate of surgery,reduce complications and mortality related.
