食管和胃粘膜延长加大网膜覆盖食管胃吻合术

目的 探讨食管癌病变食管切除后,食管胃吻合术效果较好的术式。方法 对321例食管癌采取随机分组,分别采用传统的食管胃吻合术式和食管胃粘膜延长加大网膜覆盖食管胃吻合术式进行比较。结果 传统方式154例,发生吻合口瘘7例,占4.55％。食管胃粘膜延长加大网膜覆盖食管胃吻合术式167例,无1例发生吻合口瘘。两组比较,差异有显著性(P<0.05)。结论 食管胃粘膜延长加大网膜覆盖术式能有效地预防胸内吻合口瘘及吻合口狭窄。不失为一种食管胃胸内吻合的新方法。

Anastomosis of stomach and esophagus by extending stomach -esophageal mucosa and enlarging the coverage of omentum

Objective] To study a good anastomosis of stomach and esophagus after the resection of esophagus with esophageal cancer. Methods] 321 cases of esophageal cancer were randomly divided into two groups, 154 cases were given traditional anastomosis of stomach and esophageal, others were given anastomosis of stomach and esophagus by extending stomach-esophageal mucosa and enlarging the coverage of omentum, comparison was made. Results] 7 cases were found anastomotic leak in the traditional anastomosis (4.55% ), while no such thing occurred in the new anastomosis method. The difference is significant ( P < 0.05) . Conclusion]As a new operation method, the anastomosis of stomach and esophagus by extending stomach-esophageal mucosa and enlarging the coverage of omentum is effective to avoid anastomotic leak.
