会阴侧切对初产妇母儿结局的影响

目的:比较天津市海河医院两个时期内的初产妇会阴侧切率及母儿结局,以便更合理地应用该手术。方法:回顾性病例资料分析。选取干预会阴侧切前后两个时期头位、单胎、阴道分娩的初产妇175人,组成样本,统计其会阴侧切率并记录母儿结局。比较两组会阴侧切率,分析两组之间母儿结局。结果:两个时期会阴侧切率差异有统计学意义(P<0.05),但新生儿窒息、产后出血及第二产程延长的发生率差异无统计学意义(P>0.05)。结论:干预会阴侧切前后两个时期会阴侧切率差异有统计学意义,母儿结局比较差异无统计学意义,不支持会阴侧切能够缩短产程、降低产后出血发生率和预防新生儿窒息。在初产妇正常分娩中,应加强对会阴侧切的指征控制,对于初产妇正常分娩不应常规进行会阴侧切。

Influence of lateral episiotomy to the outcomes of primiparas maternal and neonatal

Objective To compare lateral episiotomy rate and different maternal and neonatal outcomes of primiparas in two periods in a hospital so as to more reasonable application of lateral episiotomy.Method Analyzed the data of the 175 cases of primiparas with head position,singleton and normal laboring in two periods in a hospital retrospectively.And counted the rate of lateral episiotomy and different maternal and neonatal outcomes,compared the rate of lateral episiotomy and analyzed different maternal and neonatal outcomes in two periods.Results There was statistical difference between the lateral episiotomy rates of these two periods(P<0.05),but there was no statistical difference of the rate of asphyxia neonatorum,postpartum hemorrhage and prolonged second stage(P>0.05).Conclusion There was statistical difference between the lateral episiotomy rates in two periods in a hospital.There was no statistical difference of the rate of maternal and neonatal outcomes.We do not support that lateral episiotomy can decurtate the time of delivery,reduce postpartum hemorrhage and prevent asphyxia neonatorum.In view of the potential hazard of lateral episiotomy to primiparas,we should strengthen controlling lateral episiotomy indications.Routine use of episiotomy in low-risk women with normal birth is not advocated.
