胸腔镜治疗自发性气胸75例

目的 探讨电视胸腔镜手术在治疗自发性气胸中的适应证、手术方法和疗效。方法 回顾性分析该院自2002年3月～2005年12月75例自发性气胸的临床资料：VATS 71例、VAMT 2例、中转开胸手术2例。手术方式均为松解胸膜粘连＋结扎或切除肺大疤＋胸膜固定术。结果 全组无手术死亡；手术时间45～150min，平均80min；平均住院时间10d；术后带胸腔引流管平均2．5d；无手术切口感染；无复发。结论 电视胸腔镜手术具有安全可靠、有效、微创和恢复快等优点，已成为治疗自发性气胸的首选方法。

Experience of video-assisted thoracoscopic surgery for spontaneous pneumothorax (Report of 75 cases)

Objective To investigate the indications, surgical procedures and efficiency of video-assited thoracoscopic surgery in spontaneous pneumothorax. Methods The reviewing clinical materials of analysing 75 patients with spontaneous pneumothrax treated by video-assisted thoracoscopy was done in our hospital between 2002.3~2005.12 VATS 71 example. VAMT 2 example. Two cases turn to open thoracotomy. The way of operation include excising and suturying the bullae and carrying out pleurodesis. Results This group have no operation death. The main operation time was 45~150 minutes (range to 80 minutes). The average time in hospital was 10 days and intrathoracic drain retention after operation was 2.5 days. No infection in all patients. No recurrence appeared. Conclusion For spontaneous pneumothorax, video-assisted thoracic surgery (VATS) is practically efficaious and slinky with minimum trauma, fast recovery, short hospital stay and become the prefewed technique of surgical treatment.
