时间-空间相关成像技术诊断胎儿心脏畸形的应用研究

摘    要:目的:探讨应用STIC技术诊断胎儿心脏畸形的应用价值。方法:对1 286例胎儿进行超声心动图顺序检查,检查过程中应用STIC技术采集心脏容积数据,数据存盘后进行图像后处理并脱机分析,对心脏畸形的胎儿超声心动图结果,与出生后复查超声心动图及引产后病理结果进行对照分析。结果:STIC技术结合胎儿超声心动图检出胎儿心脏异常189例,其中心脏复杂畸形36例,合计91处心脏畸形,STIC技术产前诊断胎儿心脏畸形的灵敏度(97.4%),特异度(99.6%),误诊率(0.4%),漏诊率(2.6%),总的符合率(99.2%)。结论 :STIC技术结合胎儿超声心动图诊断胎儿心脏畸形具有较高的灵敏度、特异度,对胎儿心脏畸形可以明确诊断。

Abstract:Objective:To explore the application of STIC in the fetal chocardiography diagnosis of congenital heart diseases.Methods :Examined in order of fetal echocardiography for 1 286 cases,checking the application of STIC technology during the cardiac volume data acquisition and saving the image data post-processing and offline analysis,fetal echocardiography results of fetal cardiac malformations is analyzed in contrast with the results after the birth of echocardiography and pathology results.Results: The cases of 1 286,complete follow-up data in 1 080 cases,STIC technology combined with fetal echocardiography detected abnormal fetal heart 189 cases,complex cardiac malformation in which 36 patients,total of 91 heart defects,STIC technology in prenatal diagnosising of fetal cardiac malformations sensitivity(97.4%),specificity(99.6%),misdiagnosis rate(0.4%),misdiagnosis rate(2.6%),the total compliance rate(99.2%),Youden index(97%),positive predictive value(97.9%),negative predictive value(99.4%),the cases of diagnosis of fetal cardiac malformation with STIC technology,184 cases of true positive,5 cases of false-negative cases,4 cases of false-positive cases.Conclusion: STIC technology combined with fetal echocardiography can confirm the diagnosis of fetal cardiac malformations,the diagnosis of fetal cardiac malformations has high sensitivity and specificity with STIC technology,STIC technologys diagnosis of fetal cardiac malformations is the recommended method.
