铁蛋白铁含量在肝脏疾病诊断中的临床意义

目的:探讨一种测定血清铁蛋白铁含量(FIC,Ferritin iron content)的试验方法,以及FIC与临床肝脏疾病状态间的关系。方法:选择临床确诊慢性肝炎、肝炎后肝硬化各30例,原发性肝癌患者11例。采用放射免疫方法(RIA,Radio immunoassay)测定血清铁蛋白含量,将富含铁蛋白的沉淀物经NaOH变性蛋白、HCl酸化后,采用火焰原子吸收法测定上清液中铁的含量,并计算FIC。结果:FIC测定的变异系数(CV)为5.9%,平均回收率为95.1%~102.5%,溶血标本有严重干扰。肝癌组FIC显著高于其他病例组,肝硬化组显著高于慢性肝炎组和健康对照组,P<0.01。FIC与AST、ALT相关。结论:该方法准确可靠,操作简单,适合于FIC的测定。FIC能够反映肝病患者体内铁蛋白的载铁量,对疾病的诊断有较高的临床意义。

Clinical Significance of Determining Ferritin Iron Content in Patients with Liver Disease

Objective To explore a experiment method of determining ferritin iron content (FIC),and the relationship between FIC and Clinical significance in patients with liver disease.Methods 71 inpatients based on clinical were chosen,30 samples with chronic hepatitis, cirrhosis,respectively,and 11 samples with primary hepatocellular carcinoma.30 healthy person as antitheses.Serum ferritin concentrations were determined by radio immunoassay(RIA).Precipitate with enriched ferritin changed by NaOH,and acidification by HCl.Then detected the iron concentration in the supernatant by atomic absorption spectrometer.And count FIC.Results The coefficients of variation of the FIC assay were 5.9%, the average of callback were 95.1%～102.5%.Hemolytic specimen had badly interfere.The FIC in the group of hepatocellular carcinoma was more higher than in other groups,and cirrhosis group was more higher than in other groups of chronic hepatitis and health.The difference were significant,P<0.01.FIC was correlated with AST and ALT.Conclusions the technique is nicety and trustiness,operation briefness,it can be used to detecting FIC.FIC can reflect body ferritin iron overload in patients with liver disease,and it has high clinical significance to diagnoses.
