医源性胆道损伤17例处理体会

目的探讨医源性胆道损伤原因、预防及处理。方法对17例医源性胆道损伤患者的临床资料进行回顾性分析。结果本院发生3例,外院转入14例。所有病例均为胆道手术所致,11例发生于传统开腹手术,2例发生于LC,2例发生于MC,胆总管探查1例,胆管损伤后行胆管十二指肠内引流1例。术中发现2例,即行胆管修补＋T管引流。术后1周内发现14例,其中腹腔穿刺引流2例、胆管单纯修补1例、胆管修补＋T管引流1例、T管外引流＋腹腔引流1例、胆管端端吻合＋T管引流2例、去除胆管缝扎线和钛夹＋T管引流2例、胆管空肠Roux-en-Y吻合＋T管引流术2例、胆管空肠Roux-en-Y内引流3例。1例胆管十二指肠吻合术后4个月,改行胆管空肠Roux-en-Y内引流。17例经随访6月～3年均临床治愈。结论医源性胆道损伤多发生于肝外胆道手术中,胆囊切除术尤为多见。提高对医源性胆道损伤的警惕性,熟悉胆道系统的解剖及其变异,重视各种胆道疾病所发生的病理变化,认真、仔细地手术操作是预防损伤的关键。及时发现、早期正确处理对提高疗效至关重要。

Treatment of 17 Cases of Iatrogenic Bile Duct Injury
