| 摘 要： | 目的探讨微创经皮肾穿刺造瘘碎石术联合体外冲击波碎石术治疗巨大多发肾结石的可行性。方法对巨大结石92例，巨大多发结石26例，采用经皮肾穿刺造瘘碎石术＋体外冲击波碎石术＋经皮肾穿刺造瘘碎石术的“三明治”取石术。结果118例均行一期穿刺取石，单通道取石59例，双通道取石38例，三通道取石21例。体外冲击波碎石术58例，二次以上经皮肾穿刺造瘘碎石术89例。平均手术时间170min，平均住院15d。未发现明显出血等并发症。结论经皮肾穿刺造瘘碎石术＋体外冲击波碎石术＋经皮肾穿刺造瘘碎石术的“三明治”取石术是一种治疗巨大多发肾结石的有效方法。

| Abstract: | Objective] To discuss the feasibility of Percutaneous nephrolithotripsy (PCNL) combinated with Extracorporeal shock wave lithotripsy(ESWL) in treating the huge renal calculi. Methods] 92 cases of huge renal calculi and 26 cases of multiple huge renal calculi were treated by sandwich therapy(PCNL+ESWL+PCNL). Results] All cases were performed one-stage PCNL. 59 cases were treated with single-tract PCNL, 38 cases with two-tracts PCNL, and 21cases three-tracts PCNL. 58 cases combined with ESWL, 89 cases underwent two sessions PCNL. The average operation time was 170 minutes, and the mean hospitalization was 15 days, no complications like hemorrhage occurred. Conclusions] The sandwich therapy(PCNL+ESWL+PCNL) is an effective way in treating multiple huge renal calculi. |
