长期鼻饲匀浆膳食的老年患者鼻饲模式的研究

目的研究长期鼻饲匀浆膳食的老年患者最佳的鼻饲模式。方法长期鼻饲自制匀浆膳食的老年住院患者54例,随机分为实验组与对照组各27例。改进鼻饲模式每日鼻饲匀浆膳食4次,每次450ml,匀浆膳食间隔时间为5h;常规鼻饲模式每日鼻饲匀浆膳食6次,每次300ml,匀浆膳食间隔时间为3h。两组患者均观察60d,实验组患者前30d按照改进鼻饲模式进行鼻饲,后30d按照常规鼻饲模式进行鼻饲;对照组患者前30d按照常规鼻饲模式进行鼻饲,后30d按照改进鼻饲模式进行鼻饲。结果两组患者营养状况及鼻饲并发症对比均无差异(P>0.05);与常规鼻饲模式比较,改进鼻饲模式护士的鼻饲日工作时间明显减少(P<0.001),且患者在两次匀浆膳食之间的胃休息时间明显延长(P<0.001)。结论改进鼻饲模式对长期鼻饲匀浆膳食的老年患者更适宜。

Discussion of nasal feeding methods to aged patients following long period paste-formulated diet

Objective To discuss the best nasal feeding methods for aged patients receiving long period paste-formulated diet. Methods Fifty-four aged inpatients receiving long period paste-formulated diet were randomly divided into experimental group(n=27) and control group(n=27). Experimental group adopted modified mode:The patients were receiving nasal feeding 4 times a day, 450ml per time, the interval time of each meal was 5h. The control group adopted common mode:The patients were receiving nasal feeding 6 times a day, 300ml per time, the interval time of each meal was 3h. The experimental group adopted modified mode for 30 days, and then used common mode for the next 30 days. It was reversed in the control group. Results There was no significant difference for nutritional status and complications between the two groups(P>0.05). Compare with the control group, daily working time of nasal feeding in experimental group was shorter(P<0.001), patients' stomach could have more time to rest between two meals(P<0.001). Conclusions The modified nasal feeding mode was more suitable for aged patients receiving long period paste-formulated diet.
