万拉法新与西酞普兰治疗老年抑郁症对照研究

目的评价万拉法新与西酞普兰治疗老年抑郁症的临床疗效及安全性。方法将60例老年抑郁症患者随机分为两组，每组30例，研究组口服万拉法新治疗，对照组口服西酞普兰治疗，观察6w。于治疗前及治疗1w、2w、4w、6w末采用汉密顿抑郁量表评定临床疗效，副反应量表评定不良反应。结果治疗6w末，研究组总有效率93．3％，对照组为90．0％，两组总体疗效相当（P〉0．05）。研究组治疗1w末起，对照组治疗2w末起汉密顿抑郁量表总分均较治疗前有显著下降（P〈0．05或0．01）；研究组治疗1w、2w末较对照组下降更显著（P〈0．05或0．01）。两组不良反应均轻微，多发生在治疗初期，随着治疗时间的延续逐渐减轻或缓解。结论万拉法新与西酞普兰治疗老年抑郁症均有显著疗效，不良反应轻微，依从性好，但万拉法新起效更快，值得临床推广应用。

A control study of venlafaxine vs citalopram in senile depression

Objective To evaluate the efficacy and safety of venlafaxine vs citalopram in the treatment of senile depression. Methods 60 senile depression patients were randomly divided into two groups of 30 ones each,research group took orally venlafaxine and control group did citalopram for 6 weeks.Curative effects were assessed with the Hamilton Depression Scale(HAMD) and adverse reactions with the Treatment Emergent Symptom Scale(TESS) before treatment and at the end of the 1st,2nd,4th,6th week treatment. Results At the end of the 6th week,total effective rates were 93.3% in the research and 90.0% in the control group,total efficacies of two groups were equivalent(P>0.05).Total scores of the HAMD lowered more significantly since the end of the 1st week in the research and since the 2nd in the control group compared with pretreatment(P<0.05 or 0.01);so did at the end of the 1st and 2nd week in the research than in the control group(P<0.05 or 0.01).Adverse reactions of both groups were mild,took place in the initial stage of treatment mostly and gradually lessened or relieved along with time of therapy lasting. Conclusion Both venlafaxine and citalopram have significant efficacies,higher safety and better compliance, but the former takes effects faster in the senile depression.
