心电图活动平板运动试验与冠状动脉造影结果对照分析

摘    要:目的:探讨心电图活动平板运动试验对冠心病的诊断价值。方法:选择101例行冠状动脉造影检查的患者,同期内(间隔<2周)接受心电图活动平板运动试验,将其结果进行对比分析。结果:运动试验诊断冠心病的敏感性85.29%(29.34),特异性82.09%(55/67),预测准确性83.17%(84/101)。运动时间,运动耐量(最大METs)、ST段改变持续时间、冠状动脉病变组与冠状动脉造影阴性组差异明显(P<0.05,P<0.01)。双支上以冠状动脉病变组与单支冠状动脉病一对比亦有显著差异(P<0.01)。双支以上冠状动脉病变组平均运动试验阳性率高于单支病变组(P<0.05)。结论心电图活动平板运动试验是目前诊断冠心病较理想的非创伤性检查方法,通过多项指标综合判 以初步推测病变程度。

Abstract:Objective To elucidate the diagnositic value of exercise electrocardiography (EET) in coronary artery disease (CAD). Methods One hundred and one patients were examined by coronary angiography (CAG)and EET. The interval between these two procedures was less than two weeks. CAG data were compared with that of EET. Results The sensitivity, specificity, and accuracy rate in diagnosis of CAD with LET were 85.29% (29/ 34), 82.09%(55/67), and 83.17%(84/l01), respectively. There was a significant difference between the CAD group and CAG negative group in the exercise time, the maximum METs and duration of ST depression (P <0.05, P < 0.01) . There was also a significant difference between the multi-vessel disease group and the single vessel disease group (P <0.01). The patients with multi-vessel disease had a higher positive rate of exercise ECG test as compared with those with single vessel disease (P <0.05). Conclusions Exercise ECG test is an effective non-invasive method in the diagnosis of CAD. Some parameters in EET are helpful to predict the severity of CAD.
