磷酸肌酸钠和1,6-二磷酸果糖治疗小儿心肌损害效果观察

摘    要:目的观察磷酸肌酸钠和1,6-二磷酸果糖治疗小儿心肌损害的疗效。方法选择2010年10月—2013年10月因心肌损害进行药物治疗的120例患儿,随机分为对照组、磷酸肌酸钠组和二磷酸果糖组各40例。患儿入院后立即进行常规治疗,服用大量的维生素C、葡萄糖、辅酶Q10和ATP,病毒感染者进行抗病毒治疗,如患儿出现心律失常、心力衰竭应该进行及时治疗。对照组进行常规治疗,磷酸肌酸钠组加用磷酸肌酸钠,50 mg/kg溶于25 ml生理盐水中,静脉滴注,1次/d;二磷酸果糖组加用1,6-二磷酸果糖200 mg/kg溶于100 ml生理盐水中,静脉滴注,1次/d。三组均连续治疗2周。观察三组患儿治疗前后的CK、CK-MB、LDH、CTnI的变化及心电图的恢复情况。计量资料采用t检验,计数资料采用χ2检验,P0.05为差异有统计学意义。结果治疗后CK、CK-MB、LDH、CTnI均较治疗前明显下降,与对照组比较,磷酸肌酸钠组(339.86±52.90)、(0.85±0.09)、(183.22±43.32)、(13.32±6.08)U/L]和二磷酸果糖组(350.61±48.80)、(1.12±0.05)、(192.71±39.67)、(15.41±5.13)U/L)]下降更明显,差异均有统计学意义(均P0.05)。磷酸肌酸钠组和二磷酸果糖组总有效率(95.0%、92.5%)明显高于对照组,差异均有统计学意义(均P0.05)。结论磷酸肌酸钠和1,6-二磷酸果糖对小儿心肌损害具有较好疗效,可为临床用药提供参考。

Clinical effects of creatine phosphate sodium and fructose 1,6 -bisphosphate for myocardial damage in children

Abstract:Objective To observe the clinical effects of creatine phosphate sodium and fructose 1,6 - bisphosphate in the treatment of children with myocardial damage. Methods A total of 120 children receiving medical treatment for myocardial damage from October 2010 through October 2013 were randomly divided into control group, creatine phosphate sodium group and fructose 1,6 -bisphosphate group, 40 cases for each group. After admission,-conventional therapy was given to all groups, including large doses of Vit C, glucose, coenzyme Q and ATP, anti - virus therapy for viral infection, if dysrhythmias and heart failure occurred, timely treatment should be given. Control group received conventional therapy; in addition, crea- tine phosphate sodium group received intravenous administration of sodium phosphocreatine, 50 mg/kg in 25ml physical sa line, once daily; fructose 1,6- bisphosphate received intravenous administration of fructose 1,6- bisphosphate, 200 mg/kg in 100 ml physical saline, once daily. All groups were treated for 2 consecutive weeks' and observed for the level of CK,CK - MB, LDH, CTnI, and ECG. T - test was used for measurement data,x2 test for count data, the result of P 〈 0.05 was con sidered statistically significant. Results After the treatment, the level of CK, CK - MB, LDH, CTnI decreased obviously in three groups; compared with control group, creatine phosphate sodium group and fructose 1,6 -bisphosphate group decreased more obviously, the difference was statistically significant ( P 〈 0.05 ). The total effective rate was significandy higher in crea fine phosphate sodium group and fructose 1,6 -bisphosphate group than control group, the difference was statistically signifi- cant ( P 〈 0.05 ). Conclusions Creatine phosphate sodium and fructose 1,6 - bisphosphate show potent efficacy for children with myocardial damage, which provides reference for clinical medication.
