尼莫地平对重型颅脑损伤患者脑循环动力学参数的影响

摘    要:目的 观察尼莫地平对重型颅脑损伤思考脑血管血液动力学参数的影响,以指导临床应用。方法 选择80例重型颅脑损伤的患随机分为二组,常规治疗组(对照组)予以开颅手术、止血、脱水、抗炎、激素等综合治疗,尼莫地平治疗组在常规治疗的同时,加用尼莫地平治疗,连续用药达伤后30d。在伤后不同时间测定伤侧颈动脉平均血流速度、平均血流量、脑血管外周阻力和动态阻力。结果 尼莫地平治疗组的各项指标均较常规组明显提前改善,两组有显差异。结论 尼莫地平可以促使重型颅脑损伤患脑血液循环明显提前改善,从而防止继发性脑损伤的加重,促进患脑功能尽早恢复。

Abstract:Objective To observe the effect of nimodipine on cerebrovascular hemodynamics indexes in the patients with severe craniocerebral injury(SCI)and to provide a reference for the clinical use of nimodipine.Methods Eighty patients with SCI were randomly divided into two groups,i.e.conventional treatment group(control group)and nimodipine group,to which nimodipine treatment was given besides the craniotomy and conventional postoperation management which were used for treating the patients in the control group.The mean velocity and mean quantity of carotid blood flow,cerebral vascular resistance and dynamic resistance were measured at the different time after the SCI.Results Changes in all the indexes earlier were significantly ameliorated in nimodipine groups as compared to the control group(P<0.01).Conclusion The disturbances of cerebral circulation resulting in aggravation of secondary brain injury were significantly ameliorated and brain function was earlier resumed by nimodipine treatment in the patients with SCI.
