16层螺旋CT对不同肝叶灌注成像的研究

摘    要:目的:研究正常人不同肝叶血流灌注情况。方法:选择健康成人54例于肝门层面行多层动态CT灌注扫描,经软件后处理得到不同肝叶的血流动力学参数:肝血容量(BV)、肝血流量(BF)、平均通过时间(MTT)、肝动脉灌注指数(HAF),计算得到肝动脉灌注量(HAP)、门静脉灌注量(PVP)、总肝灌注量(THBF)、门脉灌注指数(PPI),并对其进行方差分析和LSD-t检验,统计软件为SPSS 13.0,检验水准α=0.05。结果:肝右叶、左内叶、左外叶的血流动力学参数分别为BV(ml/100ml):17.708±2.874、17.431±2.618、16.818±2.635;BF(ml/min/100ml):88.225±12.728、88.603±14.863、87.011±13.798;MTT(s):15.993±0.887、16.663±1.234、17.299±1.356;HAF:0.119±0.025、0.270±0.108、0.164±0.028;HAP(ml/min/100ml):12.436±4.872、20.154±6.321、14.667±3.198;pvp(ml/min/100ml):75.789±8.499、68.499±12.766、72.345±11.257;THBF(ml/min/100ml):88.225±8.695、88.603±10.097、87.011±9.426;PPI:0.881±0.017、0.730±0.073、0.836±0.193。肝左内、外叶HAF高于肝右叶,PPI低于肝右叶,肝内叶与右叶HAF及PPI差异有显著性。其余各组BV、BF、HAF、HAP、PVP、THBF、PPI差异无显著性。结论:肝左叶HAF高于肝右叶,动脉供血比例大于肝右叶;肝右叶PPI高于肝左叶,门静脉供血比例大于左叶。

Abstract:Objective To research the blood distribution in different liver lobe.Methods 54 cases of healthy adults were included.A single slice through the portal hepatis were repeatedly scanned with CT perfusion by 16 row detector spiral.The liver perfusion parameters including hepatic blood flow(BF),blood volume(BV),mean transit time(MTT),hepatic arterial fraction(HAF).hepatic artery perfusion(HAP),total hepatic blood flow(THBF),portal venous perfusion(PVP) and portal vein perfusion index(PPI)was calculated.Then the analysis of variance(ANOVA) was applied with Spss 13.0,(size of test α=0.05).Results The liver perfusion parameters of right lobe,left medial lobe and left lateral were BV(ml/100ml): 17.708±2.874,17.431±2.618,16.818±2.635;BF(ml/min/100ml): 88.225±12.728,88.603±14.863,87.011±13.798;MTT(s) : 15.993±0.887,16.663±1.234,17.299±1.356;HAF: 0.119±0.025,0.270±0.108,0.164±0.028;HAP(ml/min/100ml): 12.436±4.872,20.154±6.321,14.667±3.198;pvp(ml/min/100ml): 75.789±8.499,68.499±12.766,72.345±11.257;THBF(ml/min/100ml): 88.225 ±8.695,88.603 ±10.097,87.011 ±9.426;PPI: 0.881 ±0.017,0.730 ±0.073,0.836 ±0.193.There exists significant difference in HAF and PPI between left lobe and right lobe.As for BV,BF,MTT,HAP,PVP,THBF,there were no signficant difference among the different liver lobes.Conclusion HAF of left lobe is higher than right lobe,the proportion of arterial blood in left lobe is bigger than right lobe.PPI of right lobe is higher than left lobe,the proportion of portal vein blood in right lobe is bigger than left lobe.
