甘草酸二铵辅助治疗SAP前后血浆细胞因子的变化

目的探讨甘草酸二铵辅助治疗重症急性胰腺炎（SAP）前后血浆Ghrelin,CRP,IL-8和TNF-α浓度的变化。方法 100例急性坏死性胰腺炎患者随机分为两组：甘草酸二铵辅助治疗组和一般治疗组。入院时（0 d）和治疗3 d后进行APACHE-Ⅱ评分;入院时（0 d）、治疗1 d和3 d后,观察血淀粉酶、血脂肪酶、血浆Ghrelin、CRP、IL-8和TNF-α浓度的变化。50例门诊体检健康成年人为对照组。结果甘草酸二铵辅助治疗组和一般治疗组患者APACHE-Ⅱ评分在治疗3 d后均明显下降,甘草酸二铵辅助治疗组下降更明显（P〈0.05）。甘草酸二铵辅助治疗组和一般治疗组患者血淀粉酶、血脂肪酶、血浆Ghrelin、CRP、IL-8和TNF-α浓度均进行性下降,且甘草酸二铵辅助治疗组下降更明显（P〈0.05）。结论甘草酸二铵对SAP有明显辅助治疗作用。血浆Ghrelin、CRP、IL-8和TNF-α浓度与SAP病情严重程度相关。

Changes of plasma cytokines before and after adjunctive therapy with diammonium glycyrrhizinate in patients with SAP

Objective To discuss the changes of plasma Ghrelin,CRP,IL-8 and TNF-α before and after the adjunctive therapy with diammonium glycyrrhizinate(DG)in patients with severe acute pancreatitis(SAP).Methods 100 patients with SAP were randomly divided into two groups:DG adjunctive treatment group and normal treatment group.APACHE-Ⅱ scoring was made at the time of administration(0 d)and 3 d after the treatment.The changes of hemodiastase,lipase,plasma Ghrelin,CRP,IL-8 and TNF-α were observed at the time of administration,1 and 3 d after the treatment.Other 50 health adults were chosen as the control group.Results The APACHE-Ⅱ scores of DG adjunctive treatment group and normal treatment group all decreased significantly 3 d after the treatment,and the score of the former group decreased more obviously(P<0.05).The contents of hemodiastase,lipase,plasma Ghrelin,CRP,IL-8 and TNF-α in the two groups all decreased progressively,and those of DG adjunctive treatment group decreased more obviously(P<0.05).Conclusion DG has adjunctive therapeutic effects on SAP.The contents of plasma Ghrelin,CRP,IL-8 and TNF-α level has correlation with the severity of SAP.
