两种方法治疗髌骨骨折效果观察

目的比较改良张力带与丝线环形缝合法内固定治疗髌骨骨折的临床效果。方法 86例髌骨骨折患者随机分为观察组和对照组各43例,观察组采用改良张力带治疗,对照组采用丝线环形缝合法内固定治疗。术后3个月,采用改良膝关节活动度（ROM）评价标准,对两组进行膝关节主动活动度测定;术后6个月,对两组行关节功能远期评定。结果观察组住院时间和手术时间均短于对照组;观察组3个月后膝关节活动度总有效率为90.7%,高于对照组的72.1%;6个月后远期临床总有效率为95.3%,高于对照组的76.7%（P均〈0.05）。结论改良张力带治疗髌骨骨折操作简便,术后恢复快,治疗效果优于丝线环扎内固定法。

Effects of two methods on treatment of patellar fracture

Objective To make comparison of clinical effects between modified tension-band and internal fixation by ring-shaped silk suture on patellar fracture.Methods Eighty six cases of patellar fracture were randomly divided into observation group and control group with 43 ones in each group.The observation group was treated by modified tension-band,while the control group received internal fixation by ring-shaped silk suture.Three months after the operation,the active range of knee joint motion in both groups was detected by evaluation criterion of improved range of knee joint motion.Six months after the operation,the long term evaluation of joint function in the two groups was made.Results The length of stay and the operation time in the observation group were shorter than those in the control group.Three months after the operation,the total effective rate of the range of knee joint motion in the observation group was 90.7%,which was higher than that in the control group(72.1%).Six months after the operation,the long-term clinical total effective rate was 95.3% in the observation group,which was higher than that in the control group(76.7%)(all P<0.05).Conclusion The treatment of patellar fractures with modified tension-band has the advantages of simple operation and rapid postoperative recovery,and its curative effects are better than those of the internal fixation by silk suture cerclage.
