妊娠高血压综合征患者血浆一氧化氮、内皮素浓度的变化及其临床意义

摘    要:[目的 ]探讨一氧化氮和内皮素对妊娠高血压综合征的作用 .[方法 ]用亚硝酸盐法及放射免疫分析法测定 32例妊娠高血压综合征病人、2 0例正常妊娠妇女和 2 0例正常非孕妇女的血浆中一氧化氮的稳定代谢产物亚硝酸盐与硝酸盐比值、内皮素 1.[结果 ]①正常晚期妊娠组血浆亚硝酸盐与硝酸盐比值、内皮素 1浓度和一氧化氮与内皮素比值高于未孕正常对照组 .②中度和重度妊娠高血压综合征组与正常晚期妊娠组比较 ,各指标间均有显著性差异 .③妊娠高血压综合征病人平均动脉压与一氧化氮与内皮素比值呈负相关关系 .④伴有胎儿宫内发育迟缓组病人血浆中亚硝酸盐及硝酸盐浓度、一氧化氮与内皮素值与非胎儿宫内发育迟缓组比较均有显著性差异 .[结论 ]正常妊娠时 ,一氧化氮与内皮素比例增高 ,使血管舒张 ,是机体适应妊娠所发生的代偿性变化 ;一氧化氮与内皮素比值可作为判定妊娠高血压综合征病情程度的一项新指标

Abstract:OBJECTIVE*!To study the effects of nitric oxide(NO) and endothelin(ET) for pregnancy induced hypertension(PIH).*!METHODS*!The plasma nitrous acid/nitrate and ET 1 in 32 cases of PIH, 20 cases of normal pregnant women and 20 cases of non pregnant women were detected by nitrite reduction test and radioimmunoassay.*!RESULTS*!①The plasma nitrous acid/nitrate, ET 1 and NO/ET were apparently higher in normal pregnant group than in normal non pregnant group; ②There were significant changes of all indexes between moderate, severe PIH groups and normal pregnant group; ③There was apparently negative correlation between mean arterial pressure and NO/ET; ④The plasma nitrous acid/nitrate, NO/ET were lower and ET 1 was higher in intrauterine growth retardation group than in non intrauterine growth retardation group, but no significant changes.*!CONCLUSION*!The increase of NO/ET results in the diastole of blood vessel that is a compensatory change of organism to fit for pregnancy. The occurrence of PIH is possibly related to the decrease of NO and the increase of ET. The imbalance of NO/ET enables serious consequence to fetal growth. The NO/ET is a new index which might judge patient's condition in PIH.
