乙肝病毒外膜大蛋白临床应用价值研究

目的检测乙肝患者血清中乙肝病毒外膜大蛋白（LHBs），并与乙肝病毒脱氧核糖核酸（HBVDNA）和乙肝病毒前S1抗原（PreS1Ag）相比较，探讨LHBs的临床应用价值。方法采用酶联免疫吸附实验（ELISA）检测140例乙肝患者血清中LHBs，乙肝病毒标志物（HBV—M）、HBV PreS1Ag，同时采用荧光定量聚合酶链式反应（PCR）方法对血清HBV DNA进行检测，并进行统计分析。结果在所研究的各种模式中，LHBs与HBV DNA的检出率差异均无统计学意义（P均〉0．05）并且LHBs的总阳性率（76．43％）高于PreSiAg阳性率（44．29％）和乙肝e抗原（HBeAg）的阳性率（29．29％），差异有统计学意义（P均〈0．05）。结论乙肝病毒外膜大蛋白在一定程度可以反映HBV感染者体内病毒复制情况。其作为HBV复制指标，灵敏度高于PreS1Ag和HBeAg，可作为判断HBV感染者体内乙肝病毒复制情况新的血清学指标，尤其是针对HBeAg阴性患者。

Clinical significance of serum large surface protein of hepatitis B virus

Objective To study the clinical significance of serum large surface protein of hepatitis B virus （LHBs）. Methods Contents of HBV DNA were determined by fluorescent quantitative PCR. Serum levels of LHBs, HBV - M, and preS1 antigen were examined by ELISA in 140 serum samples. Results There was no significant difference between positive rates of LHBs and HBV DNA in this study. There was significant difference among positive rates of LHBs , PreS1 and HBeAg, and the positive rate of LHBs was higher than that of PreS1 and HBeAg respectively. Conclusion The level of serum LHBs may be served as a reliable marker in reflection of HBV replication , especially in HBeAg - negative HBV infected patients.
