外固定支架术治疗桡骨远端关节内骨折的临床研究

目的：观察外固定支架术治疗桡骨远端关节内移位骨折的临床疗效。方法：80例桡骨远端关节内骨折患者根据手术方法分为外固定支架术（治疗组）和手法整复石膏外固定术（对照组），对术后骨折愈合时间、愈合质量评分、两组腕关节功能及术后并发症进行评价。结果：治疗组骨折愈合质量评分显著低于对照组，相比较有显著性差异（t=16．608，P〈0．01）；但两组骨折愈合时间比较差异无统计学意义（t=1．183，P〉0．05）。治疗组腕关节功能评估优良率为90．0％，显著高于对照组（52．5％），两相相比较有显著性差异（X2=13．730，P〈O．01）。观察组术后并发症发生率为12．5％，对照组为75．0％，两组相比较有显著性差异（X2=31．746，P〈0．01）。结论：对于桡骨远端关节内骨折，特别是移位明显且不稳定骨折，提倡选择外固定支架固定术治疗。

Clinical Study of External Fixation Technique in Treating the Patients with Intra-articular Distal Radius Fractures

Objective： To study clinical efficacy of external fixation technique in treating the patients with intra-articular distal radius fractures. Method： 80 cases of patients with intra-articular distal radius fractures were divided into manual reduction and plaster immobilization （ control group } and external fixation technique （treatment group} according to surgical methods. After surgery the fracture healing time, healing quality score, rehabilitation of wrist function and postoperative complications were evaluated. Result： The healing quality score of treatment group was significantly lower than the control group, compared the differ- ence was significant（t= 16. 608,P〈0.01 }, and the fracture healing time of two group was no significantly difference（ t= 1. 183 ,P〉0.05 }. The good rate of rehabilitation of wrist function of treatment group was 90. 0%, it was significantly higher than that of the control group （52.5% }, the difference was significant（ X2 = 13. 730 ,P〈0.01 ） ; The postoperative complications of treatment group was 12.5%, it was significantly lower than that of the control group （ 75.0% ）, the difference was significant （ X2 = 31. 746, P〈0.01 }. Conclu- sion： For intra- articular distal radius fractures, especially displaced and unstable fractures obviously should be encouraged to choose external fixation.
