血清甲状腺激素水平对慢性肝硬化患者治疗疗效影响研究

目的:探讨肝硬化对血清甲状腺激素水平的影响及其临床意义。方法:检测80例肝硬化患者TT3、TT4、FT3、FT4、TSH的含量,并与对照组80例上述指标对照,肝硬化患者按Child-Pugh方法分级为A、B、C三级,各级之间进行对比。肝硬化患者按治疗效果不同,比较不同各组之间血清甲状腺激素水平。结果:肝硬化组患者入院时TT3、FT3、TT4、FT4降低,TSH升高,对照组TT3、FT3、TT4、FT4、TSH正常,两组比较差异具有统计学意义(P<0.05),血清甲状腺激素水平随肝硬化病变程度的加重与对照组比较差异越显著(P<0.05)。慢性肝硬化患者治疗疗效较好者血清甲状腺激素水平得到改善。结论:测定甲状腺激素水平可以作为评估肝硬化的严重程度及预后等具有重要的参考价值。

Change of serum thyroid hormone levels in chronic liver cirrhosis patients

Objective: To explore the change serum thyroid hormone levels in chronic liver cirrhosis patients and its clinical significance.Methods: A total of 80 cirrhosis patients and another 80 controls were selected.Serum level of triio-dothyronine(TT3),thyroid hormone(TT4),free triiodothyronine(FT3),free thyroid hormone(FT4) and thyrotropin hormone(TSH) were determined and compared.Cirrhosis patients were divided into A,B,C grade according Child-Pugh.The levels of thyroid hormone in serum were measured and compared.Results: TT3,FT3,TT4,FT4 of cirrhosis patients were decreased while TSH was obviously increased when admitting to hospital;while in the controls,TT3,FT3,TT4,FT4,TSH were normal.The differences between two groups were significant(P<0.05).The abnormality of thyroid hormone level was aggravated as the aggravating state of cirrhosis(P<0.05).Thyroid hormone level was more significantly improved in patients with more significant therapeutical efficacy.Conclusions:Thyroid hormone level can be used an index for predicting the severity and prognosis of cirrhosis.
