固尔苏联合鼻塞式持续气道正压通气治疗新生儿呼吸窘迫综合征的疗效观察

目的探讨固尔苏联合鼻塞式持续气道正压通气(NCPAP)治疗新生儿呼吸窘迫综合征(NRDS)的临床疗效。方法 72例NRDS患儿按数字表随机分为两组,对照组(n=36例)在综合治疗措施上采用沐舒坦联合NCPAP治疗,观察组(n=36例)在综合治疗措施上采用固尔苏联合NCPAP治疗。观察两组患儿临床疗效,治疗前后临床表现,比较两组患儿治疗后1、12、24h NCPAP参数(Fi O2、PIP、PEEP、Flow)、动脉血气指标(p H、Pa O2、Pa CO2、Pa O2/Fi O2)的变化。结果观察组治疗总有效率(88.9%)明显高于对照组治疗总有效率(72.2%)(P0.05);随着治疗时间延长,两组患儿动脉血气指标和NCPAP参数均得到改善,且观察组动脉血气指标和NCPAP参数(Fi O2、PIP)改善较对照组更为显著(P0.05)。结论固尔苏联合NCPAP治疗NRDS患儿疗效显著,可明显改善动脉血气指标和NCPAP参数,值得临床推广应用。

Observation of curative effect of curosurf combined with nasal continuous positive airway pressure in the ;treatment of neonatal respiratory distress syndrome

Objective To study the curative effect of curosurf combined with nasal continuous positive air-way pressure （ NCPAP） in the treatment of neonatal respiratory distress syndrome （ NRDS） . Methods 72 children with NRDS were randomly divided into two groups. The control group （n=36） was treated with mucosolvan com-bined with NCPAP on comprehensive treatment measures, and the observation group （n=36） was treated with curo-surf combined with NCPAP on comprehensive treatment measures. The clinical curative effect, clinical symptoms be-fore and after the treatment of the two groups were observed, and the changes of NCPAP parameters （ FiO2、PIP、PEEP、Flow）, arterial blood gas index （pH、PaO2、PaCO2、PaO2/FiO2） in the two groups 1, 12, 24h after the treat-ment were compared. Results The total effective rate of the observation group （ 88. 9%） was significantly higher than that of the control group（72. 2%） （P〈0. 05）. With treatment prolonged, the arterial blood gas index and NC-PAP parameters of the two groups were improved, and the improvement was more pronounced in the observation group than in the control group （P〈0. 05）. Conclusion Curosurf combined with NCPAP has a good curative effect in the treatment of NRDS, which can significantly improve the arterial blood gas index and NCPAP parameters.
