髋臼内壁截骨全髋关节置换术治疗髋臼发良不良伴股骨头坏死

目的探讨髋臼内壁截骨全髋关节置换术在发育不良髋关节髋臼重建中的应用。方法采用结合髋臼内壁截骨术的全髋关节置换治疗髋关节发育不良患者11髋，男7例，女4例，平均年龄56岁，臼杯置于真性髋臼水平．平均随访时间11．7个月。采用改良Harris评分对结果进行评估。术前及随访时进行摄片观察。结果所有人工臼杯均安置于真臼位置，术后随访3．个月X线片提示截骨部位均骨性愈合良好，假体无移位．患髋功能良好。结论内壁截骨术有助于将发育不良髋关节的髋臼安置于真臼位置．保留髋臼底部的骨量．避免髋臼外上方植骨的并发症，近期手术效果良好。

Total hip arthroplasty with circumferential acetabular medial wall osteotomy and cementless cup for coxarthrosis due to dysplastic hip

Objective To evaluate the outcomes of total hip arthroplasty for coxarthrosis due to dysplasia with circumferential acetabular medial wall osteotomy and cementless cup.Methods A retrospective study was made on 11 hips （7 male and 4 female;averaged age 56 years）with developmental hip dysplasia treated by THA with circumferential acetabular medial wall osteotomy and cementless cup .The acetabular cup was placed at the level of the true acetabulum.The average follow-up period was 11.7 months （ranged,3- 23 months）. All patients were evaluated with the use of a modified Harris hip score.Radiographic evaluations were made by preoperative and follow-up.Results All the acetabular cups were placed at the level of the true acetabulum.In the cases followed-up more than 3 months,the osteomized site all achieved bone union. According to the modified Harris hip score,the average hip score increased from 48 at preoperation to 88 at the final review.No complications such as infection, dislocation,prosthesis loosening.Conclusion THA with an uncemented cup and circumferential acetabular medial wall osteotomy for coxarthrosis due to dysplasia could obtain favorable results. This method could restore acetabular bone stock in patients and avoid the complications due to bone-graft.
