基底动脉尖综合征的临床及影像学改变的分析

目的探讨基底动脉尖综合征的临床特点、影像学改变及早期诊断等问题。方法观察10例基底动脉尖综合征患者的临床表现和头颅MRI改变,以及2例全脑血管造影(DSA)改变。结果10例患者分别出现不同程度的意识障碍、眼球运动障碍、瞳孔异常和偏盲等,头颅MRI显示双侧丘脑、脑干、小脑、枕叶等处多发脑梗死灶,2例DSA结果示在基底动脉顶端血管狭窄。结论根据患者临床表现,及早进行头颅MRI扫描对早期诊断、治疗及预后有重要意义,DSA有确诊价值。

Clinical manifestations and iconographic changes of top of the basilar syndrome

Objective To study some problems related to clinical manifestations,iconographic changes and early diagnosis of top of the basilar syndrome(TOBS).Methods Clinical manifestations and changes in head MRI in 10 cases of TOBS and changes in DSA in 2 cases were observed.Results Ten cases presented with disorders of consciousness,oculomotor abnormalities,pupillary abnormalities,and hemianopsia to various degrees and so on.Head MRI in 10 cases showed multiple infarcts in the thalamus,brainstem,cerebellum and occipital lobes.DSA in 2 cases showed stenosis of top of the basilar artery.Conclusion Early head MRI scan is extremely important to early diagnosis,treatment and predicting prognosis in patients with TOBS.DSA is of great value for confirming diagnosis.
