大段同种异体骨对下颌骨节段性缺损修复临床探讨

目的探讨临床上应用大段同种异体骨进行下颌骨节段性缺损修复的临床价值和效果。方法对12例因下颌骨良性肿瘤行下颌骨切除后形成的下颌骨节段性缺损,选用与之大小形态相配的大段深冷冻辐射同种异体骨进行缺损性修复,恢复下颌骨的形态及功能。结果 12例病例术后创口愈合良好,牙合关系恢复满意,术后面部形态基本对称,经术后随诊1～2年,移植骨无明显的骨吸收表现,无1例发生骨不愈及排斥性反应导致同种异体骨移植失败。结论同种异体骨在下颌骨节段性缺损具有良好的骨形态结构,能恢复下颌骨的自然形态及弧度,移植骨无明显的骨吸收和骨不愈表现,能重建恢复下颌骨原有的形态和功能,是下颌骨缺损重建较理想的修复材料,临床治疗效果满意。

The Clinical Exploration to Reconstruct Mandibular Segmental Defects by Using Massive Bone Allografts

Objective To explore the clinical value and effect of applying massive bone allografts to reconstruct the mandibular segmental defects. Methods 12 patients with mandibular benign tumor were excised the sects of mandibular. The operation made part of mandibular defects . To reconstruct the defects and renew the configuration and the function use as the same shapes of freeze - drying and radiate massive bone allografts as the patients. Results After the operation, the wounds of the 12 patients coalesced well, the occlude relation renewed satisfied and basically their visages are symmetical. Also follow up these cases 1-2 years, their transplanting bones have not absorbed obviously. No one case such as the bone didn' t coalesce or immunologic rejection makes the transplanting of allografts fail down. Conclusions Allograft has good configuration to mandibular segmental defects.It also can renew the normal configuration and the radian of mandibular.The transplanting bones didn' t show any situations like bone unabsorbed or unhealed up. It can reconstruct the former configuration and function of mandibular. It is comparatively good restoring materials for mandibular defectively reconstruction.The effect clinical treatment is well.
