超声弹性成像硬度评分法与面积比值法在乳腺肿块鉴别诊断中的应用

摘    要:目的探讨超声弹性成像评分法与面积比值法在乳腺肿块鉴别诊断中的临床应用价值。方法对158个乳腺肿块行弹性成像检查,先用超声弹性成像评分法进行评分,然后分别测量弹性图像和二维图像上肿块的面积,计算其面积比。所有肿块均获得病理结果。结果超声弹性成像评分法诊断乳腺良恶性肿块的敏感性、特异性、准确性分别为93.9%、92.1%、92.4%,面积比值法诊断乳腺良恶性肿块的敏感性、特异性、准确性分别为84.0%、87.1%、71.1%。两种方法联合诊断的敏感性、特异性、准确性分别为93.9%、98.4%、97.5%。结论联合应用超声弹性成像评分法与面积比值法可提高鉴别乳腺良恶性肿块的准确性,具有较高的临床应用价值。

Abstract:Objective To explore the clinical value of improved-5 scoring system and area ratio of ultrasound elastography in the differential diagnosis of breast lesions. Methods A total of 158 lesions underwent ultrasound elastography and hardness scoring with improved-5 scoring system. The area in the elastographic image and B-mode image were measured and the area ratio was then calculated. The pathological results of all lesions were obtained. Results The sensitivity, specificity and accuracy of improved-5 scoring system in the diagnosis of breast lesions were 93.9% ,92.1% ,92.4%, respectively. For area ratio method, they were 84.0%, 87.1% ,71.1%, respectively. For the combination of improved -5 scoring and area ratio, they were 93.9% ,98.4% ,97.5% ,respectively. Conclusion The combination of scoring and area ratio can improve the accuracy of ultrasound elastography in the differential diagnosis of breast lesions and has a high clinical value.
