单纯硬脊膜外海绵状血管瘤的MRI诊断

摘    要:目的 探讨单纯硬脊膜外海绵状血管瘤的MRI诊断价值。方法 回顾分析 5例硬脊膜外海绵状血管瘤患者的MRI图像。MRI平扫和增强检查在 1.0T超导磁场中完成 ,采用快速自旋回波序列。结果 肿块均位于胸段脊髓背侧 ,呈卵圆形 ,MRI表现为T1WI呈等或低信号 ,T2WI呈高信号 ,Gd DTPA增强后明显强化 ,肿块与脊髓间有条状低信号硬脊膜间隔 ,提示病灶位于硬脊膜外腔。结论 MRI能清晰显示肿块的部位、范围和信号改变 ,一般能在术前作出诊断 ,目前属最佳检查方法。

Abstract:Objective To investigate the value of magnetic resonance imaging (MRI) in the diagnosis of pure spinal epidural cavernous hemangioma. Methods Five cases of pure spinal epidural cavernous hemangioma with MRI examination were revieved and analysed. MRI examination was performed with a 1.0 tesla superconductive magnetic field, and processed with turbo spin echo sequence. Results All tumors which had an oval shape were localized in the dorsal thoracispinal cord. Magnetic resonance imaging showed low or intermediate signal on T1WI and high signal on T2WI, tumor was enhanced markedly after contrast injection. There was a low signal intensity band between the lesion and the spinal cord.This low intensity band was considered to suggest the tumor's locating extradural position. Conclusion MRI can well display the location,extent and signal changes of the tumor,and allows a presumptive presurgical diagnosis.So far, MRI is the best method in the diagnosis of pure spinal epidural cavernous hemangioma.
